摘要
目的探讨基于DRGs付费理念的绩效变革在医院精细化管理中的应用效果。方法通过统计分析贵港市某三级甲等综合医院2017—2019年的数据,以DRGs付费理念为指导,重新制定绩效考核方案。采集2020—2021年的HIS系统信息数据,应用第三方公司绩效软件系统、EXCEL电子表格进行统计分析,按照新绩效考核方案进行绩效分配并评估其应用效果。结果基于DRGs付费理念的绩效方案变革,医院运行状况良好。与2020年对比,2021年该院诊疗人次增长13.8%,CMI值提高16%,四级手术增长43.6%,微创手术增长30.6%,药品耗材占比降低6.2%,医疗服务性收入增长7.1%,运行效率有效增长。结论基于DRGs付费理念的绩效变革,有助于推进医院各项工作的精细化管理,促进医院高质量发展。
Objective To explore the application effect of performance reform based on DRG-based payment on hospital fine management.Methods Through statistical analysis of 2017—2019 data of a tertiary hospital in Guigang City,under the guidance of DRGs payment,new performance appraisal was formulated.The information data of HIS system from 2020—2021 were collected,and the performance software system of third-party companies and EXCEL were used for statistical analysis.According to the new scheme of performance appraisal,performances were distributed in details and its application effect was evaluated.Results After performance reforms based on DRG-based payment,the hospital was running well.Compared with the data of 2020,various medical indicators of the hospital improved in 2021:the medical visits increased by 13.8%,the CMI value increased by 16%,the level IV surgery increased by 43.6%,the minimally invasive surgery increased by 30.6%,the proportion of pharmaceutical consumables decreased by 6.2%,the medical service income increased by 7.1%,and the operation efficiency increased effectively.Conclusion The performance reforms based on the DRG-based payment leads to the promotion of hospital fine management and high-quality development.
