胃镜联合腹腔镜治疗对老年胃间质瘤患者的影响

Effect of Gastroscopy Combined with Laparoscopy Treatment on Elderly Patients with Gastric Stromal Tumor

目的:分析胃镜联合腹腔镜治疗对老年胃间质瘤患者的影响。方法:回顾性选取2015年1月-2021年7月莆田学院附属医院收治的72例老年胃间质瘤患者。将接受开腹手术治疗的患者归为参照组,接受胃镜联合腹腔镜治疗的患者归为试验组,每组36例。比较两组围手术期指标、术后恢复情况、并发症、病理检测及免疫组化结果及复发风险。结果:试验组手术时间长于参照组,术中出血量少于参照组,住院时间短于参照组(P<0.05)。试验组独自下床活动时间、拔除胃管时间、首次肛门排气时间均早于参照组(P<0.05)。试验组并发症发生率为2.78%,低于参照组的19.44%(P<0.05)。两组CD117^(+)、CD34^(+)、DOG-1^(+)率及良恶性率比较差异均无统计学意义(P>0.05)。两组极低风险、低度风险、中度风险占比比较差异均无统计学意义(P>0.05)。结论:针对胃间质瘤的治疗,胃镜联合腹腔镜治疗具有多方面的优势,体现出可行性与安全性。

Objective:To analyze the effect of gastroscopy combined with laparoscopy treatment on elderly patients with gastric stromal tumor.Method:A total of 72 elderly patients with gastric stromal tumors treated in Affiliated Hospital of Putian University from January 2015 to July 2021 were selected retrospectively.Patients treated with open surgery were included in the reference group,and patients treated with gastroscopy combined with laparoscopy were included in the experimental group,with 36 cases in each group.The perioperative indicators,postoperative recovery condition,complications,pathological examination and immunohistochemical results and recurrence risk of the two groups were compared.Result:The operation time of the experimental group was longer than that of the reference group,the amount of intraoperative bleeding was less than that of the reference group,and the hospital stay was shorter than that of the reference group(P<0.05).The activity time of getting out of bed alone,the time of removing the drainage tube and the first anal exhaust time in the experimental group were earlier than those in the reference group(P<0.05).The incidence of complications in the experimental group was 2.78%,which was lower than 19.44%in the reference group(P<0.05).There were no significant differences in the rates of CD117^(+),CD34^(+),DOG-1^(+)and benign and malignant rates between the two groups(P>0.05).There were no significant differences in the proportions of very low risk,low risk,medium risk between the two groups(P>0.05).Conclusion:For the treatment of gastric stromal tumors,gastroscopy combined with laparoscopy has many advantages in many ways,reflecting the feasibility and safety.
