摘要
目的:基于二维斑点追踪技术(2D-STE)探讨苓桂术甘汤改善慢性心力衰竭大鼠心肌功能及心室重构机制。方法:选取44只SPF级SD大鼠,连续4周腹腔注射阿霉素,建立慢性心力衰竭大鼠模型。将造模成功的SD大鼠随机分为空白组、模型组、地高辛组、苓桂术甘汤组进行观察。结果:地高辛组、苓桂术甘汤组左室射血分数(LVEF)、左室短轴缩短率(FS)、左心室室间隔二维应变参数(绝对值)、二维斑点纵向应变绝对值(GLS)均升高,左室舒张末内径(LVIDd)、左室收缩末内径(LVIDs)降低,差异有统计学意义(均P<0.05)。地高辛组、苓桂术甘汤组血浆B型脑钠肽(BNP)、血管紧张素Ⅱ(Ang-Ⅱ)及内皮素1(ET-1)降低,差异有统计学意义(均P<0.05)。地高辛组ET-1、室间隔各节段纵向应变参数(绝对值)、GLS均较苓桂术甘汤组高,差异有统计学意义(均P<0.05)。GLS与BNP、Ang-Ⅱ及ET-1呈正相关性。结论:苓桂术甘汤可改善慢性心力衰竭大鼠心肌功能及心室重构,且与血清学指标相符。
Objective:To investigate efficacy of Linggui Zhugan decoction in mechanism of improving myocardial function and ventricular remodeling in chronic heart failure rats based on 2D-STE.Methods:44 SPF-grade SD rats were selected and injected intraperitoneally with adriamycin for 4 weeks to establish rat model of chronic heart failure.The successfully modeled SD rats were randomly divided into blank group,model group,digoxin group and Linggui Zhugan decoction group for observation.Results:The left ventricular ejection fraction(LVEF),fraction shortening(FS),left ventricular septal two-dimensional strain parameter(absolute value),and two-dimensional speckle longitudinal strain absolute value(GLS)were increased and the left ventricular end-diastolic internal diameter(LVIDd)and left ventricular end-systolic internal diameter(LVIDs)decreased in the the digoxin group and the Linggui Zhugan decoction group,difference statistically significant(all P<0.05).Plasma B brain natriuretic peptide(BNP),angiotensinⅡ(Ang-Ⅱ)and endothelin 1(ET-1)were reduced in the the digoxin group and the Linggui Zhugan decoction group,difference statistically significant(all P<0.05).ET-1,longitudinal strain parameters(absolute values)and GLS were higher in the digoxin group than those in the Linggui Zhugan decoction group,difference statistically significant(all P<0.05).GLS was positively correlated with BNP,Ang-Ⅱand ET-1.Conclusion:Linggui Zhugan decoction can significantly improve myocardial function and ventricular remodeling in rats with chronic heart failure.The results are consistent with serological indexes.
