摘要
目的分析多模态磁共振成像(MRI)技术在颅内肿瘤中的临床应用价值,为临床诊断提供参考。方法选取2018年12月至2020年12月南充市高坪区人民医院收治的74例颅内肿瘤患者为研究对象进行回顾性分析。以病理结果为金标准,分析多模态MRI诊断符合率,并比较颅内不同肿瘤的表观扩散系数(ADC)值、相对脑血容量(rCBV)、胆碱/肌酸(Cho/Cr)及N-乙酰天门冬氨酸/肌酸(NAA/Cr)值。结果MRI检查诊断符合率与病理检查结果具有良好一致性(P<0.05)。根据病理结果将74例患者分为胶质瘤组、转移瘤组及脑脓肿组,3组患者环壁ADC值、坏死区rCBV值比较,差异无统计学意义(P>0.05),胶质瘤组患者环壁、水肿区rCBV值均明显高于转移瘤组和脑脓肿组(P<0.05);胶质瘤组、转移瘤组患者的坏死区ADC值明显高于脑脓肿组(P<0.05);胶质瘤组患者水肿区ADC值低于脑脓肿组、转移瘤组(P<0.05),胶质瘤组患者环壁、水肿区Cho/Cr值高于转移瘤组,转移瘤组高于脑脓肿组(P<0.05);胶质瘤组患者环壁、水肿区NAA/Cr值低于转移瘤组,转移瘤组低于脑脓肿组(P<0.05)。结论多模态MRI在鉴别诊断颅内肿瘤中具有重要价值,可提高诊断效率,为临床治疗制定提供可靠依据,值得临床应用。
Objective To analyze the clinical application value of multimodal magnetic resonance imaging(MRI)technology in intracranial tumors to provide reference for clinical research.Methods A total of 74 patients with intracranial tumors admitted to Nanchong Gaoping District People’s Hospital from December 2018 to December 2020 were selected as the study subjects for retrospective analysis.Taking the pathological results as the gold standard,the diagnostic coincidence rate of multimodal MRI was analyzed,and the apparent diffusion coefficient(ADC)value,cerebral blood volume(rCBV),Choline(CHO)/Creatine(Cr)and N-acetylaspartic acid(NAA)/Cr values of different intracranial tumors were compared.Results The diagnostic coincidence rate of MRI examination was in good agreement with the pathological examination results(P<0.05).According to the pathological results,74 patients were divided into the glioma group,the metastatic tumor group and the brain abscess group,there was no significant difference in the ADC value of the ring wall and the rCBV value of the necrotic area in patients among the three groups(P>0.05).The rCBV values of annular wall and edema area in glioma group were significantly higher than those in metastasis tumor groups and brain abscess group(P<0.05).The ADC value of necrotic area in glioma group and metastasis tumor group was significantly higher than that in brain abscess group(P<0.05).The ADC value of the edema area in the glioma group was lower than that in the brain abscess group and the metastatic tumor group(P<0.05).The Cho/Cr values of the ring wall and edema area in the glioma group were higher than those in the metastatic tumor group,and the metastases tumor group was higher than the brain abscess group(P<0.05).The NAA/Cr values in the ring wall and edema area of the glioma group were lower than those in the metastatic tumor group,and the metastatic tumor group was lower than the brain abscess group(P<0.05).Conclusion Multimodal MRI has important value in differential diagnosis of intracranial tumors,which can improve the diagnostic efficiency and provide a reliable basis for clinical treatment,and is worthy of cinical application.
