肩锁韧带联合改良双TightRope袢钢板重建喙锁韧带对RockwoodⅢ型肩锁关节脱位效果的影响

Effect of acromioclavicular ligament combined with modified double TightRope loop plate to reconstruction of coracoclavicular ligament on RockwoodⅢacromioclavicular joint dislocation

目的探讨肩锁韧带联合改良双TightRope袢钢板重建喙锁韧带对RockwoodⅢ型肩锁关节脱位效果的影响。方法选取2018年4月至2020年6月于江苏省无锡市太湖医院就诊的肩锁关节脱位患者82例,按照随机数字表法分为对照组(41例,接受改良双TightRope袢钢板重建喙锁韧带治疗)和研究组(41例,在对照组基础上联合肩锁韧带治疗)。比较两组相关康复指标,比较两组手术前后Constant-Murley评分、视觉模拟评分法(VAS)评分,观察两组并发症的发生情况。结果研究组手术时间长于对照组,术中出血量高于对照组,术后引流量少于对照组(P<0.05)。术后1年,两组Constant-Murley评分高于术前,且研究组高于对照组(P<0.05)。术后6个月,两组VAS评分低于术前,且研究组低于对照组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论RockwoodⅢ型肩锁关节脱位患者采用肩锁韧带联合改良双TightRope袢钢板重建喙锁韧带治疗效果确切,可减轻肩关节疼痛,且安全可靠。

Objective To investigate the effect of acromioclavicular ligament combined with modified double TightRope loop plate to reconstruct coracoclavicular ligament on RockwoodⅢacromioclavicular joint dislocation.Methods A total of 82 patients with acromioclavicular joint dislocation from April 2018 to June 2020 in Wuxi Taihu Hospital of Jiangsu Province were selected.They were divided into control group(41 cases,who received modified double TightRope loop plate to reconstruction of coracoclavicular ligament)and study group(41 cases,combined with acromioclavicular ligament treatment on the basis of control group).The related rehabilitation indicators between two groups were compared,Constant-Murley scores and visual analogue scale(VAS)scores before and after operation were compared between two groups,and the incidence of complications was observed.Results The operation time of study group was longer than that of control group,the intraoperative blood loss was higher than that of control group,while the postoperative drainage volume was shorter than that of control group(P<0.05).One year after operation,the Constant-Murley scores of two groups were higher than those before operation,and those of study group was higher than those of control group(P<0.05).At six months after operation,the VAS scores of two groups were lower than those before operation,and those of study group was lower than those of control group(P<0.05).There was no significant difference in the incidence of complications between two groups(P>0.05).Conclusion The treatment of Rockwood typeⅢacromioclavicular joint dislocation by acromioclavicular ligament combined with modified double TightRope loop plate to reconstruction of coracoclavicular ligament is effective,can reduce shoulder pain,and is safe and reliable.
