摘要
目的探讨医用弹力袜对改善下肢原发性静脉曲张患者静脉血流动力学的临床价值。方法选取2017年10月至2019年10月河北省沧州中西医结合医院收治的110例下肢原发性静脉曲张患者,根据随机抽签法将其分为对照组(n=55,采用常规非手术方式进行治疗)和观察组(n=55,在对照组基础上应用医用弹力袜)。比较两组患者治疗后的临床疗效、凝血功能指标[国际标准化比值(INR)、D-二聚体(D-D)、纤维蛋白原(FIB)]和下肢血流动力学指标(股深静脉、腘静脉、髂外静脉血流速度),比较两组患者治疗期间的不良反应发生情况。结果观察组患者总有效率为89.09%,高于对照组患者的72.73%,差异有统计学意义(P<0.05)。治疗后,两组患者INR均高于本组治疗前,FIB、D-D水平均低于本组治疗前,且观察组患者INR高于对照组患者,FIB、D-D水平均低于对照组患者,差异均有统计学意义(P<0.05)。治疗后,两组患者股深静脉、腘静脉、髂外静脉血流速度均高于本组治疗前,且观察组患者上述指标均高于对照组患者,差异均有统计学意义(P<0.05)。观察组患者不良反应总发生率为21.82%,高于对照组患的14.55%,但差异无统计学意义(P>0.05)。结论医用弹力袜能较好地改善下肢静脉曲张患者静脉血流动力学指标,提高患者的治疗效果,安全性较高,值得临床推广。
Objective To explore the clinical value of medical elastic socks on improving venous hemodynamics in patients with primary varicose vein of lower extremity.Method A total of 110 patients with primary varicose vein of lower extremity who were treated in the Cangzhou Hospital of Integrated TCM-WM·Hebei from October 2017 to October 2019 were selected and randomly divided into the observation group and control group,with 55 cases in each group.The control group was treated by conventional non-surgical method,while the observation group was treated by medical elastic socks on the basis of the control group.After treatment,the clinical effect,coagulation parameters of international normalized ratio(INR),D-dimer(D-D),and fibrinogen(FIB),lower extremity hemodynamics of femoral venous blood,popliteal vein,and external iliac vein blood flow velocity of two groups were compared.The adverse reactions between the two groups during treatment were compared.Result The total effective rate of the observation group was 89.09%,higher than 72.73%in the control group,the difference was statistically significant(P<0.05).After treatment,the INR in the two groups was higher than that before treatment,the levels of FIB and D-D in the two groups were lower than those before treatment,and the of INR in the observation group was higher than that of control group,the levels of FIB and D-D in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the blood flow velocity of deep femoral vein,popliteal vein and external iliac vein were higher than those before treatment,and the blood flow velocity of those in the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 21.82%,higher than 14.55%in the control group,but the difference was not statistically significant(P>0.05).Conclusion Medical elastic socks can obviously improve the hemodynamics in patients with primary varicose vein of lower extremity,enhance the therapeutic effect with higher safety,which is worthy of promotion.
