快速康复护理联合中药熏洗对高位复杂性肛瘘患者肛门功能的影响

Effect of rapid rehabilitation nursing combined with traditional Chinese medicine fumigation on anal function of patients with high complex anal fistula

目的探讨快速康复护理联合中药熏洗对高位复杂性肛瘘患者肛门功能及炎症反应的影响。方法选取2019年7月至2020年9月南华大学附属第二医院收治的96例高位复杂性肛瘘患者为研究对象,采用随机数字表法等分为对照组(48例)和观察组(48例)。对照组患者采用快速康复护理进行干预,观察组则在对照组护理方式的基础上采用中药熏洗护理进行干预。比较2组的肛门功能、创面渗液量、肉芽组织评分及炎症因子水平。结果术后7 d和10 d,2组的肛门失禁评分均比术后2 d低,创面渗液量均比术后2 d少(P均<0.05),且观察组均比同期对照组更优(P均<0.05)。观察组在术后7 d和10 d的肉芽组织评分分级均比同期对照组更优(P均<0.05)。术后7 d、10 d,2组的肿瘤坏死因子-α(TNF-α)和白介素-6(IL-6)水平均比术后2 d更低;术后10 d,2组的TNF-α和IL-6水平均比术后7 d更低;观察组的TNF-α和IL-6水平均比同期对照组更低,均存在统计学差异(P均<0.05)。结论快速康复护理联合中药熏洗可改善高位复杂性肛瘘患者的肛门功能,降低炎症因子水平,缓解临床症状。

Objective To investigate the effect of rapid rehabilitation nursing combined with traditional Chinese medicine fumigation on the anal function and inflammatory response of patients with high complex anal fistula.Methods A total of 96 patients with high complex anal fistula admitted to the Second Hospital,University of South China from July 2019 to September 2020 were recruited and equally divided into a control group(48 cases)and an observation group(48 cases)by random number table method.The control group was intervened with rapid rehabilitation nursing,while the observation group was intervened with traditional Chinese medicine fumigation based on the nursing method of the control group.The anal function,wound exudate,granulation tissue scores,and inflammatory factor levels were compared between the two groups.Results On the 7th and 10th day after operation,the anal incontinence scores of the two groups were lower than those on the 2nd day after operation,and the amount of wound exudate was less than that on the 2nd day after operation(all P<0.05).The observation group was better than the control group on the corresponding period(all P<0.05).The granulation tissue grade of the observation group on the 7th and 10th day after operation was better than that of the control group on the corresponding period(both P<0.05).On the 7th and 10th day after operation,the levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the two groups were lower than those on the 2nd day after operation(all P<0.05).On the 10th day after operation,the levels of TNF-αand IL-6 in the two groups were lower than those on the 7th day after operation(all P<0.05).The levels of TNF-αand IL-6 in the observation group were lower than those in the control group on the corresponding period(all P<0.05).Conclusion Rapid rehabilitation nursing combined with traditional Chinese medicine fumigation can improve the anal function of patients with high complex anal fistula,reduce the levels of inflammatory factors,and relieve the clinical symptoms.
