冻融胚胎移植与新鲜胚胎移植对体外受精-胚胎移植患者临床妊娠结局的影响比较

Effects of frozen thawed embryo transfer and fresh embryo transfer on clinical pregnancy outcomes in patients with in vitro fertilization and embryo transfer

目的比较冻融胚胎移植与新鲜胚胎移植对体外受精-胚胎移植(IVF-ET)患者临床妊娠结局的影响。方法选取60例行IVF-ET患者,根据胚胎不同的类型将患者分为冻融胚胎移植组与新鲜胚胎移植组,每组30例。冻融胚胎移植组进行冻融胚胎移植;新鲜胚胎移植组取新鲜胚胎后立即种植。比较两组排卵情况、着床成功率及妊娠情况。结果两组患者促性腺激素(Gn)初始计量比较,差异无统计学意义(P>0.05);冻融胚胎移植组人绒毛膜促性腺激素(HCG)日雌二醇(E_(2))水平高于新鲜胚胎移植组,Gn总用量、HCG日孕酮(P)、HCG日卵泡数及获卵数均低于新鲜胚胎移植组,差异有统计学意义(P<0.05)。冻融胚胎移植组的着床成功率96.67%、临床妊娠率93.33%均高于新鲜胚胎移植组的73.33%、73.33%,异位妊娠率3.33%低于新鲜胚胎移植组的26.67%,差异有统计学意义(P<0.05)。结论冻融胚胎移植能有效提高植入成功率和临床妊娠率,降低异位妊娠率,使其获得更好的妊娠结局。

Objective To compare the effects of frozen thawed embryo transfer and fresh embryo transfer on clinical pregnancy outcomes in patients with in vitro fertilization embryo transfer(IVF-ET).Methods 60 patients with IVF-ET were selected and divided into frozen thawed embryo transfer group and fresh embryo transfer group according to different types of embryos,with 30 cases in each group.Frozen thawed embryo transfer was performed in the frozen thawed embryo transfer group;Fresh embryos in the fresh embryo transfer group were harvested and immediately implanted.Ovulation,implantation success rate and pregnancy were compared between the two groups.Results There was no significant difference in the initial measurement of gonadotropin(Gn)between the two groups(P>0.05);The human chorionic gonadotropin(HCG)daily estradiol(E_(2))level in the frozen thawed embryo transfer group was higher than that in the fresh embryo transfer group,and the total dosage of Gn,progesterone(P)on the HCG day,the number of follicles and oocytes retrieved on the HCG day were lower than that in the fresh embryo transfer group,the difference was statistically significant(P<0.05).The success rate of implantation and clinical pregnancy rate of frozen thawed embryo transfer group were 96.67%and 93.33%respectively,which were higher than 73.33%and 73.33%of fresh embryo transfer group,and the ectopic pregnancy rate of 3.33%was lower than 26.67%of fresh embryo transfer group.The difference was statistically significant(P<0.05).Conclusion Embryo transfer with frozen thawing can effectively improve transplantation and clinical pregnancy rates,reduce ectopic pregnancy rates,and achieve better results during pregnancy.
