摘要
目的观察普瑞巴林胶囊联合冰敷标准化作业对三叉神经痛射频热凝后患者氧化应激反应、舒适度的影响。方法回顾性选取2018年10月至2019年10月沧州市人民医院收治的100例因三叉神经痛行射频热凝术治疗患者为研究对象,根据冰敷方法不同分为两组,对照组50例术后采用普瑞巴林胶囊联合常规冰敷干预,观察组50例术后采用普瑞巴林胶囊联合冰敷标准化作业干预。比较两组疼痛、肿胀、舒适度评分的差异,检测两组炎症因子[C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)、红细胞沉降率(ESR)]、氧化应激因子[超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)]的变化,统计两组术后累计并发症发生率。结果术后2 d,两组患者疼痛、肿胀评分均明显较术后1 d升高,到术后3 d,两组疼痛、肿胀评分较术后1 d下降,且观察组术后1、2、3 d的疼痛、肿胀评分均明显低于对照组,差异均有统计学意义(P<0.05)。术后3 d,观察组舒适量表总分及其生理、心理、精神、社会和环境4个维度的评分均明显高于对照组,差异均有统计学意义(P<0.05)。术后,两组CRP、PCT、IL-6、ESR均较术前下降,SOD、GSH-Px均较术前升高,观察组术后CRP低于对照组,SOD、GSH-Px、PCT、IL-6、ESR与对照组比较,差异均无统计学意义(P>0.05)。观察组术后累计并发症发生率为4.00%,明显低于对照组(16.00%),差异有统计学意义(P<0.05)。结论普瑞巴林胶囊联合冰敷标准化作业可减轻三叉神经痛射频热凝后疼痛、肿胀程度,提高患者舒适度,减轻机体炎症反应和氧化应激损伤,减少并发症的发生。
Objective To observe the effects of pregabalin capsules combined with standardized ice compress on oxidative stress and comfort in patients with trigeminal neuralgia after radiofrequency thermocoagulation.Methods A total of 100 patients with trigeminal neuralgia who received radiofrequency thermocoagulation in Cangzhou People's Hospital from October 2018 to October 2019 were retrospectively selected as the research objects,and divided into two groups according to different ice compress methods.Pregabalin capsules combined with conventional ice compress intervention,and 50 cases in the observation group were treated with pregabalin capsules combined with ice compress standardized operation intervention after operation.The differences in pain,swelling and comfort scores between the two groups were compared,the changes of inflammatory factors[C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6),and erythrocyte sedimentation rate(ESR)]and oxidative stress factors[superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)]were detected in the two groups,and the cumulative incidence of postoperative complications in the two groups was calculated.Results On the 2nd day after the operation,the pain and swelling scores of the two groups were significantly higher than those on the 1st day after the operation,and on the 3rd day after the operation,the pain and swelling scores in the two groups decreased compared with the 1st day after the operation,the pain and swelling scores of the observation group at 1 d,2 d,and 3 d after operation were significantly lower than those in the control group,the differences were statistically significant(P<0.05).On the 3rd day after operation,the total score of the comfort scale and the scores of four dimensions of physiology,psychology,spirit,society and environment in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).After operation,the levels of CRP,PCT,IL-6,and ESR in the two groups were all decreased compared with those before operation,the levels of SOD,GSH-Px were higher than those before operation,and the level of CRP after operation in the observation group was lower than that in the control group,but there was no statistical difference in SOD,GSH-Px,PCT,IL-6 and ESR(P>0.05).The cumulative incidence of postoperative complications in the observation group was 4.00%,which was significantly lower than that in the control group(16.00%),the difference was statistically significant(P<0.05).Conclusion Pregabalin capsules combined with standardized ice compresses can reduce pain and swelling after radiofrequency thermocoagulation in trigeminal neuralgia,improve patient comfort,reduce body inflammatory response and oxidative stress injury,and reduce the occurrence of complications.
