围透析期慢性病管理模式的构建与研究

Construction and Research on Management Mode of Chronic Diseases in Peridialysis Period

目的:观察慢性病管理模式在围透析期患者管理中的应用效果。方法:选取2020年1月-2022年4月广东江门市新会区新会人民医院肾内风湿科收治的慢性肾脏病1~3期患者78例为观察对象,根据随机数字表法分为对照组和观察组,各39例。对照组实施常规护理模式干预,观察组实施围透析期慢性病管理模式干预,比较两组患者生活质量评分、自我护理能力及并发症发生率。结果:观察组生理职能、生理机能、社会功能、躯体疼痛、情感职能、活力、精神健康及一般健康情况评分高于对照组,差异有统计学意义(P<0.05);观察组自我护理能力量表中自护技能、自护责任感、自我概念及健康认知水平评分高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:将慢性病管理模式用于围透析期患者管理中,有助于改善患者生活质量,提升自我护理能力,预防并发症。

Objective:To observe the application effect of chronic disease management mode in the management of patients in peridialysis period.Methods:A total of 78 patients with stage 1~3 chronic kidney disease in the Department of Nephrology and Rheumatology,Xinhui People's Hospital of Xinhui District in Jiangmen of Guangdong Province from January 2020 to April 2022 were selected as the observation objects.They were divided into control group and observation group according to random number table method,with 39 cases in each group.The control group implemented routine nursing mode,and the observation group implemented chronic disease management mode in the peridialysis period.The quality of life scores,self-care ability and complication rates of the two groups were compared.Results:The scores of physical role,physical function,social function,somatic pain,emotional role,vitality,mental health and general health in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The scores of self-care skills,self-care responsibility,self-concept and health cognition level on the self-care ability scale in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of chronic disease management mode in the management of patients in the peridialysis period is helpful to improve the quality of life of patients,improve self-care ability,and prevent the complications.
