热敏灸对脾胃虚弱证泄泻型肠易激综合征的影响

The effects of heat sensitive moxibustion on putients with diarrhea predominant iriable bowel syndrome of spleen stomach deficiency syndrome

目的探讨热敏灸对脾胃虚弱证泄泻型肠易激综合征(IBS-D)患者的近期及远期疗效影响。方法将60例脾胃虚弱证IBS-D患者随机分为观察组和对照组,每组各30例。两组均进行基础西医治疗;对照组采用固定穴行温和灸;观察组在全腹探查热敏点行热敏灸。观察两组近期及远期疗效。结果两组患者治疗后第30天IBS-D症候积分均降低(P<0.05);观察组治疗后症候积分较对照组降低(P<0.05),有效例数分别为26例和19例,差异有统计学意义(P<0.05)。复发病例数及症候积分差异无统计学意义(P>0.05)。结论热敏灸可降低脾胃虚弱证IBS-D患者症候积分,改善症状;但不能减少IBS-D的复发及改善远期预后,可为脾胃虚弱证IBS-D患者早期治疗提供一种新思路。

Objective To investigate the short-term and long-term effects of heat sensitive moxibustion on patients with diarrhea predominant irritable bowel syndrome(IBS-D)of spleen stomach deficiency syndrome.Methods Sixty IBS-D patients with spleen stomach weakness syndrome were randomly divided into intervention group and control group,with 30 cases in each group.Both groups were given basic western treatment.The control group was treated with Shenjue,Tianshu(both sides),Shangjuxu,Zusanli(both sides),Pishu and Weishu.Three of the above acupoints were selected for mild moxibustion.In the intervention group,through exploring the whole abdomen,the location of heat sensitive points was found and marked and heat sensitive moxibustion was carried out.The short-term and long-term efficacy of the two groups were observed.Results Compared with before treatment,IBS-D symptom scores of the two groups were lower on the 30th day fter treatment(P<0.05).Compared with the control group,the IBS-D syndrome score of the intervention group was lower than that of the control group(P<0.05).The effective cases of the intervention group and the control group were 26 cases and 19 cases respectively,and the difference was statistically significant(P<0.05).After 4 weeks of follow-up,there was no significant difference in the number of recurrent cases and IBS-D syndrome score between the intervention group and the control group(all P>0.05).Conclusion Heat sensitive moxibustion can reduce the IBS-D syndrome score of IBS-D patients with spleen and stomach deficiency syndrome,and improve the symptoms of IBS-D patients with spleen and stomach deficiency syndrome.Although heat sensitive moxibustion cannot reduce the recurrence of IBS-D and improve the long-term prognosis,it provides anovel thinking to make an early reasonable protocol of treatment.
