基于数据挖掘甘爱萍教授治疗慢性胃炎用药规律分析

An analysis of Professor GAN Aiping’s medication rule in treating chronic gastritis based on data mining

目的:借助中医传承辅助平台整理归纳甘爱萍教授治疗慢性胃炎的用药规律,进而能够一定程度上指导临床用药。方法:收集2017年1月―2018年1月湖北省中医院花园山院区老年病科甘教授治疗慢性胃炎的处方,将其录入中医传承辅助平台。采用频数、关联及聚类分析等方法挖掘甘教授治疗慢性胃炎的用药规律。结果:共收集到处方125个,其中包含的中药有158味。从药物性味归经及频次可以看出,甘教授使用最多的药物为甘味、淡味、平味药,归经以归肺、脾、胃、心、肝经为主;使用频次较高的药物有茯苓、百合、蒲公英、砂仁、炒白术、石斛、浙贝母、白及、佛手、玫瑰花等。从药物功效来看,以补虚药为主,其次为理气药,还涉及利水渗湿药。从药物组合来看,甘教授常用的药物组合有炒白术-茯苓、砂仁-百合、蒲公英-茯苓等。从聚类分析可以看出药物核心组合有4个,其中1、2组合聚合较好,3、4组合聚合较差。从功效上看,1、2组合偏于健脾理气,3、4组合偏于健脾化湿。结论:甘爱萍教授治疗慢性胃炎时用药平和,强调“忌刚用柔”。她所用的药物多为药食两用之品,性味甘淡,药性平和。同时,甘教授认为“脾胃为本,调气为先”,故她常在健脾益胃的药物中加疏肝理气、养心安神的药物。

Objective:The medication rules of Professor GAN Aiping in the treatment of chronic gastritis are sorted out and summarized with the help of the inheritance auxiliary platform of traditional Chinese medicine,so as to guide the clinical use of drugs to a certain extent.Methods:The prescriptions of Professor GAN Aiping of Geriatrics in the Garden Hospital of Hubei Provincial Hospital of Traditional Chinese Medicine from January 2017 to January 2018 were collected and recorded into the inheritance assistant platform of traditional Chinese medicine.The methods of frequency,correlation and cluster analysis were used to explore the medication rule of Professor GAN in the treatment of chronic gastritis.Results:A total of 125 prescriptions were collected,including 158 TCM medicines.Most of the drugs used by Professor GAN were sweet,light and flatastes,and the meridians were mainly lung,spleen,stomach,heart and liver meridiuns;the frequently used drugs were Poria Cocos,lilies,dandelion,Amomum villosum,fried Atractylodes Ovata,Dendrobium,Fritillariae Thunbrgii Bulbus,Bletilla Striata,bergamot,rose and so on.From the perspective of drug efficacy,the main drugs were deficiency-tonifying drugs,which was followed by Qi(气)-regulating drugs,also related to diuresis and dampness medicine.From the perspective of drug combination,Professor GAN’s commonly used drug combinations were fried Atractylodes OvataPoria Cocos,Amomum villosum-lily,Dandelion-Poria and so on.In the cluster analysis,it can be seen that there were 4 core combinations of drugs,among which the polymerization of 1 and 2 was better,and that of 3 and 4 was poor.In terms of efficacy,combination 1 and 2 tended to invigorate the spleen and regulate Qi,while combination 3 and 4 tended to invigorate the spleen and dissolve dampness.Conclusion:Professor GAN Aiping applies mild drugs in the treatment of chronic gastritis.She stresses that“Avoiding hardness and using softness”.Most of the drugs she used are for both medicine and food,with light taste and mild property.At the same time,Professor GAN believes that“Spleen and stomach are the foundation and regulating Qi is the first”,so she often combines the drugs for soothing the liver and regulating Qi,nourishing the heart and calming the mind with the drugs of invigorating the spleen and stomach.
