摘要
目的分析2019-2021年重庆市MSM新发现472例HIV-1感染者分子网络特征,为精准干预相关人群中HIV-1传播提供实验室依据。方法收集重庆市疾病预防控制中心性病门诊2019-2021年新报告的MSM感染者血浆样本,扩增pol区基因并测序,构建该人群HIV-1传播分子网络,采用χ^(2)检验进行成簇相关因素分析。结果从580例感染者中,共获得pol区基因序列472条。472例感染者分别属于9种基因亚型,主要亚型为CRF07_BC(344,72.88%)和CRF01_AE(96,20.34%),其他亚型包括CRF55_01B(18,3.81%)、B(4,0.85%)、CRF08_BC(6,1.27%)等。在基因距离0.8%时,共产生32个分子簇,入网率27.33%(129/472),以CRF07_BC入网最高,占82.95%(107/129)。结论MSM中新报告HIV-1感染者病毒基因呈现多样性,并存在传播活跃者。应对传播活跃者实施精准干预,以降低该人群的新发感染率。
Objective To analyze the molecular transmission network characteristics among 472 newly HIV-1 infected individuals from the MSM in Chongqing from 2019 to 2021,providing laboratory support for tailored intervention of HIV-1 transmission in the related population.Methods Plasma samples were collected from 2019 to2021 among newly reported HIV-1 infected MSM that visited the STDs Clinic of Chongqing CDC.Fragments of the pol gene were amplified and sequenced and the molecular transmission network among this population was constructed.Factors associated with transmission clusters were analyzed with the Chi-square test.Results Among the 472 pol gene sequences obtained from 580 newly infected patients.The 472 infected patients belonged to 9 subtypes.The main subtypes were CRF07_BC(344,72.88%),CRF01_AE(96,20.33%),along with other subtypes of fewer frequencies including CRF55_01B(18,3.81%),B(4,0.85%),and CRF08_BC(6,1.27%),etc.Under the genetic distance of 0.8%,a total of 32 molecular clusters were generated,resulting in a clustering rate of 27.33%(129/472).The highest clustering rate was CRF07_BC,accounting for 82.95%(107/129).Conclusions Newly reported HIV-1 infection among MSM showed viral gene diversity and identified putative high-risk spreaders,so as to implement precise intervention and reduce HIV-1 infection rate in this population.
