高频电刀肛瘘根治术联合地奥司明治疗肛瘘患者的效果及对术后疼痛、并发症的影响

The Effect of High-frequency Electroknife Radical Anal Fistula Resection Combined with Diosmin in the Treatment of Patients with Anal Fistula and Its Influence on Postoperative Pain and Complications

目的:探讨肛瘘根治术联合地奥司明治疗肛瘘患者的效果及对术后疼痛、并发症的影响。方法:选取2019年2月-2022年2月经东台市中医院确诊的74例肛瘘患者中,根据就诊先后顺序从1~74号编号,其中1~37号纳入对照组,38~74号纳入研究组,两组均予以高频电刀肛瘘根治术。对照组术后常规予以中药熏洗坐浴及凡士林软膏换药,观察组在对照组基础上予以地奥司明。比较两组创面愈合情况、疼痛、创面水肿评分、并发症发生情况和复发率。结果:研究组总有效率为97.30%,比对照组的78.38%高,差异有统计学意义(P<0.05)。研究组术后3、7 d的视觉模拟评分法(VAS)评分和创面水肿程度评分均较对照组低,差异均有统计学意义(P<0.05)。研究组并发症发生率为2.70%,低于对照组的21.62%,差异有统计学意义(P<0.05)。两组复发率比较,差异无统计学意义(P>0.05)。结论:高频电刀肛瘘根治术与地奥司明联合用于肛瘘治疗,不仅能够促进创面愈合,减轻水肿,缓解疼痛症状,还能降低并发症发生率。

Objective: To investigate the curative effect of high-frequency electroknife radical anal fistula resection combined with Diosmin in the treatment of patients with anal fistula and its influence on postoperative pain and complications. Method: A total of 74 patients with anal fistula diagnosed in Dongtai Hospital of Traditional Chinese Medicine from February 2019 to February 2022 were selected, numbered from 1 to 74 according to the order of treatment, among which No. 1 to 37 were included in the control group and No. 38 to 74 were included in the study group, both groups were given high-frequency electroknife radical anal fistula resection. The control group was given traditional Chinese medicine fumigation and sitting bath and Vaseline ointment dressing change,and the observation group was given Diosmin on the basis of the control group. Wound healing, pain, wound edema scores, complications and recurrence rate were compared between the two groups. Result: The total effective rate of the study group was 97.30%, which was higher than 78.38% of the control group, the difference was statistically significant(P<0.05). Visual analog scale(VAS) scores and wound edema degree scores of the study group were lower than those of the control group at 3 and 7 d after surgery, the differences were statistically significant(P<0.05). The complication rate of the study group was 2.70%, which was lower than 21.62% of the control group, the difference was statistically significant(P<0.05). There was no significant difference in recurrence rate between the two groups(P>0.05). Conclusion: The combination of high-frequency electroknife radical anal fistula resection and Diosmin in the treatment of anal fistula can not only promote wound healing, reduce edema, relieve pain symptoms, but also reduce the incidence of complications.
