肩袖撕裂患者行关节镜下双锁定环技术缝合治疗的临床疗效

Clinical Efficacy of Arthroscopic Double Locking Ring Technique Suture Treatment in Patients with Rotator Cuff Tears

目的探讨关节镜下双锁定环技术缝合治疗肩袖撕裂患者的临床治疗效果。方法随机选取2019年8月—2020年7月山东国欣颐养集团新汶中心医院治疗的60例肩袖撕裂患者为研究对象,按照手术方式将其分为研究组和参照组,每组30例,参照组采用关节镜下单排锚钉缝合固定进行肩袖修复,研究组采用关节镜下双锁定环技术缝合治疗进行肩袖修复,对比两组治疗前后的肩关节功能评分、肩关节外展上举活动度以及VAS评分和临床治疗效果。结果术前,两组ASES评分和Constant-Mueley评分比较,差异无统计学意义(P>0.05);术后,研究组ASES评分和Constant-Mueley评分[(89.51±4.61)分、(79.89±3.64)分]明显高于参照组[(78.51±3.61)分、(69.11±2.35)分],差异有统计学意义(t=10.290、13.628,P<0.05)。术前,两组患者VAS评分比较,差异无统计学意义(P>0.05),术后3个月、术后6个月及术后12个月,研究组VAS评分均明显低于参照组,差异有统计学意义(t=5.132、5.209、4.129,P<0.05)。术前、术后两组患者肩关节上举外展活动度比较,差异无统计学意义(P>0.05)。研究组临床疗效优良率明显高于参照组,差异有统计学意义(χ^(2)=5.104,P<0.05)。结论关节镜下双锁定环技术缝合治疗肩袖撕裂具有创伤小,疗效好,缝合简单、牢固等优点,促进了术后早期肩功能锻炼。

Objective To investigate the clinical efficacy of arthroscopic double locking ring technique suture treat⁃ment for patients with rotator cuff tears.Methods 60 patients with rotator cuff tears treated were randomly selected in Xinwen Central Hospital,Shandong Guoxin Yiyang Group from August 2019 to July 2020 were selected as the study objects,according to the surgical method,they were divided into study group and reference group,30 cases in each group,the reference group used arthroscopic single row anchor suture fixation for rotator cuff repair,the study group used arthroscopic double locking ring technique suture treatment for rotator cuff repair,shoulder function scores,shoulder abduction range of motion,VAS scores and clinical treatment effects were compared between the two groups before and after treatment.Results Before surgery,there was no statistically significant difference between the two groups of ASES scores and Constant-Mueley scores(P>0.05);after surgery,the ASES score and Constant-Mueley score[(89.51±4.61)points and(79.89±3.64)points]were significantly higher than the reference group[(78.51±3.61)points and(69.11±2.35)points],the difference was statistically significant(t=10.290,13.628,P<0.05).Before surgery,there was no statistically significant difference between the two groups of the VAS scores(P>0.05),the VAS scores of the study group were significantly lower than the reference group at 3 months,6 months and 12 months after the study group,the difference was statistically significant(t=5.132,5.209,4.129,P<0.05).Comparison of extension of abduction of the shoulder before and after surgery between the two groups,the difference was not statistically significant(P>0.05).The excellent clinical efficacy rate of the study group was significantly higher than that of the reference group,the difference was statistically significant(χ^(2)=5.104,P<0.05).Conclusion Arthroscopic double locking ring technique suture treat⁃ment of rotator cuff tears has the advantages of small trauma,good efficacy,simple and firm suture,and promotes early postoperative shoulder functional exercise.
