精细化护理模式联合中药封包治疗在CCU心脏瓣膜病患者静脉血栓干预效果

Intervention Effect of fine Nursing Mode combined with Traditional Chinese medicine package therapy on Venous Thrombosis in Patients with CCU Heart Valve Disease

目的研究探讨精细化护理模式联合中药封包治疗在CCU心脏瓣膜病患者静脉血栓干预效果。方法选取2019年9月—2021年9月医院心内科收治的106例入住CCU心脏瓣膜病伴静脉血栓患者分为观察组、对照组,各53例。对照组在CCU治疗期间增加精细化护理模式进行护理,观察组在完成对照组精细化护理干预同时给予中药封包治疗。评估比较所有患者的凝血指标、血液流变学、生活质量、护理满意度。结果观察组D-二聚体(D-D)、纤维蛋白原(FIB)水平低于对照组,活化部分凝血活酶时间(APTT)水平高于对照组(P<0.05)。治疗干预后观察组患者的高切全血黏度(HBV)、中切全血黏度(MBV)、低切全血黏度(LBV)水平均低于对照组(P<0.05)。治疗干预后观察组患者六分钟步行距离、步频、明尼苏达心力衰竭生活质量评分高于对照组(P<0.05)。出院时观察组患者评估的护理满意度高于对照组(P<0.05)。结论精细化护理模式联合中药封包治疗在CCU心脏瓣膜病患者静脉血栓干预效果良好,能够降低患者的血液高凝状态,促进患者的血流动力学的提升,提高患者的六分钟步行距离和步频,改善患者的预后生活质量,提高患者满意度,可在临床实践中推广应用。

Objective To investigate the effect of fine nursing mode combined with traditional Chinese medicine package therapy on venous thrombosis in patients with CCU heart valve disease.Methods 106 patients with valvular heart disease and venous thrombosis admitted to the Department of Cardiology of the First Affiliated Hospital of Henan University of Science and Technology from September 2019 to September 2021 were selected and divided into the observation group and the control group,with 53 patients in each group.During CCU treatment,the control group added fine nursing mode for nursing,while the observation group completed the fine nursing intervention of the control group and gave herbal medicine package treatment.The coagulation index,hemorheology,quality of life and nursing satisfaction of all patients were evaluated and compared.Results:the observation group levels of D-Dimer(D-D)and fibrinogen(FIB)were lower than control group,and the level of activated partial thromboplastin time(APTT)was higher than that in the control group(P<0.05).The observation group levels of high cut whole blood viscosity(HBV),medium cut whole blood viscosity(MBV)and low cut whole blood viscosity(LBV)lower than those in the control group(P<0.05).After treatment intervention,6 MWD,gait frequency and Minnesota Heart Failure Quality of life scale(mlhfq)in the observation group were higher than those in the control group(P<0.05).At discharge,the nursing satisfaction of the observation group was higher than that of the control group(P<0.05).Conclusion The fine nursing mode combined with traditional Chinese medicine package therapy in CCU patients with valvular heart disease has a good effect in the intervention of venous thrombosis.It can reduce the patients’blood hypercoagulability,promote the improvement of patients’hemodynamics,improve the patients’six minute walking distance and pace frequency,improve the patients’quality of life in advance,and improve the patients’satisfaction with nursing,which can be popularized in clinical practice.
