2020-2021年河北省新诊断HIV-1感染者中和抗体分布状况

Neutralizing antibodies in patients newly diagnosed with HIV-1 infection in Hebei Province in 2020-2021

摘要 目的了解河北省2020-2021年新诊断HIV-1感染者不同人群中和抗体分布特征及潜在的影响因素。方法收集河北省2020年-2021年新诊断的HIV-1感染者血浆共500份,中和实验筛查感染者血浆中和抗体分布,PCR扩增和测序获得感染者体内毒株env基因序列,分析gp120的V1V2区、V3区和gp41基因氨基酸序列长度及非同义突变与同义突变的比例(dN/dS)所体现的选择压力与感染者血浆中和广度的相关性。结果相较于异性恋感染者,男男同性恋感染者群体中和抗体滴度更高,异性恋感染者群体中,男性感染者的中和抗体滴度高于女性感染者;CRF01_AE和BC重组型感染者血浆样本对同亚型假病毒表现出更强的中和活性(P均<0.05),而单纯B亚型和C亚型毒株感染者血浆样本对同亚型假病毒或不同亚型假病毒的中和活性均无统计学差异;gp120的V1V2区的氨基酸序列长度及体现选择压力的dN/dS均与中和抗体的中和广度线性相关。结论HIV-1感染者体内中和抗体在不同传播人群分布存在差异,男男同性恋感染者人群比异性恋人群具有更高的中和活性,重组型感染者血浆展现出对同亚型假病毒的中和优势;且中和抗体的中和广度与病毒gp120高变区的氨基酸长度及宿主的选择压力密切相关。 OBJECTIVE To investigate the distribution characteristics of neutralizing antibodies in patients newly diagnosed with HIV-1 infection in Hebei Province in 2020-2021 and observe the potential influencing factors.METHODS Totally 500 plasma samples were collected from the patients who were newly diagnosed with HIV-1 infection in Hebei Province from 2020 to 2021.The distribution of neutralizing antibodies in the plasma of the infected patients was screened by neutralization test.The env gene sequence of the infected patients was obtained by PCR amplification and sequencing.The correlation between the amino acid sequence length of V1V2 region,V3 region of gp120 and gp41 gene,selection pressure reflected by nonsynonymous mutation(dN)/synonymous mutation(dS)and the plasma neutralization breadth of the infected persons was observed.RESULTS The neutralizing antibody titer of the MSM infected patients was higher than that of the heterosexual infected patients.Among the heterosexual infected patients,the neutralizing antibody titer of the male infected patients was higher than that of the female infected patients.The plasma samples of the patients with CRF01_AE and BC recombinants infection showed more powerful neutralizing activity against the same subtype of pseudovirus(all P<0.05),while there was no significant difference in the neutralizing activity against the same subtype of pseudovirus or different subtypes of pseudovirus between the plasma samples of the patients with simple B subtype infection and the plasma samples of the patients with simple C subtype infection.The length of amino acid sequence in V1V2 region of gp120 and dN/dS that reflected the selection pressure showed linear association with the neutralization width of the neutralizing antibodies.CONCLUSION There is significant difference in the distribution of neutralizing antibodies of the patients with HIV-1 infection among different transmission populations.The neutralizing activity of the MSM infected patients is higher than that of the heterosexual infected patients.The patients with recombinant infection show the neutralizing advantage of plasma against the same subtype of pseudovirus.The neutralization width of neutralizing antibodies is closely associated with the length of amino acid in hypervariable region of gp120 and selection pressure of the host.
