Effect of PDCA Nursing on Antiviral Treatment Adherence and Efficacy in Hepatitis B Patients

目的 探索PDCA护理方法对乙肝患者抗病毒治疗依从性及疗效的影响。方法 以我院2022年1月至2022年12月收治的慢性乙型肝炎进行口服抗病毒药治疗的患者200例作为研究对象,采用PDCA护理措施,即对患者存在的问题通过P(plan)计划、D(do)实施、C(check)检查、A(action)处理4个步骤采取相应的措施,并以持续改进的方法对患者进行护理。采用MMAS-8评价表调查问卷,将问卷制作成问卷星形式给患者进行在线填写,护理干预前后各填1次表,每人只有1次填写提交机会。护理前后各抽血检测1次患者肝功能。以MMAS-8评价表各项评分和总分、谷丙转氨酶(ALT)、总胆红素(TBIL)为评价指标,统计护理干预前后患者依从性和肝功能的变化。结果 与护理干预前相比,PDCA护理后患者MMAS-8评价表各项评分和总分均有显著升高,谷丙转氨酶(ALT)水平显著降低,差异具有统计学意义(P <0.05)。结论 PDCA护理干预能有效提高乙型肝炎患者口服抗病毒药治疗的依从性,能显著改善患者肝功能,提升治疗效果。

Objective To explore the effect of PDCA nursing method on the compliance and efficacy of antiviral therapy in patients with hepatitis B.Methods A total of 200 patients with chronic hepatitis B treated with oral antiviral drugs in our hospital from January 2022 to December 2022 were selected as the research objects, and PDCA nursing measures were adopted. That is, the problems of patients through P(plan) plan, D(do) implementation,C(check) check, A(action) to take corresponding measures, and continue to improve the method of patient care. The MMAS-8 evaluation questionnaire was used, and the questionnaire was made into questionnaire star form for the patients to fill out online. Before and after nursing intervention, the questionnaire was filled out once, and each person had only one chance to fill in and submit. Blood samples were drawn to detect liver function before and after nursing. The scores and total scores of MMAS-8 evaluation table, alanine aminotransferase(ALT) and total bilirubin(TBIL) were used as evaluation indexes to analyze the changes of patients’ compliance and liver function before and after nursing intervention. Results Compared with before nursing intervention, the scores and total scores of MMAS-8 evaluation table of patients after PDCA nursing were significantly increased, and alanine aminotransferase(ALT) level was significantly decreased(P<0.05). Conclusion PDCA intervention can effectively improve the compliance of oral antiviral therapy in patients with hepatitis B, significantly improve the liver function of patients, and enhance the treatment effect.
