会阴侧切连续缝合与间断缝合效果及对产妇切口恢复的影响

Effect of continuous suture and interrupted suture of perineal lateral incision of women and their influence on the recovery of the incision

目的:探究会阴侧切连续缝合与间断缝合的效果及对切口恢复的影响。方法:以2019年3月-2021年3月于本院顺产且进行会阴侧切的141例产妇为研究对象,使用随机数字表法分两组,分别行连续缝合(连续组,n=71)或间断缝合(间断组,n=70),两组均随访3个月,比较切口缝合、愈合、切口疼痛、住院等时间差异,分析两组分娩3 d后切口愈合情况,产后3个月时评估两组性功能[女性性功能量表(FSFI)],比较两组住院及随访期间切口不良反应发生。结果:连续组会阴切口缝合时间(15.5±2.7 min)、愈合时间(5.9±2.0 d)、切口疼痛时间(2.3±0.6 d)、住院时间(5.8±1.1 d)均低于间断组(20.1±2.6 min、7.3±1.9 d、3.0±0.7 d、6.8±1.6 d),分娩3 d后连续组切口愈合质量优于间断组;产后3个月时连续组FSFI量表各维度得分均高于间断组(均P<0.05),产后两组切口线头反应、瘢痕触痛、感染发生率无差异(P>0.05),皮下硬结发生率连续组(2.8%)低于间断组(11.4%)(P<0.05)。结论:会阴侧切连续缝合操作时间短,更有利于产后会阴切口愈合及性功能恢复,且减少缝合后不良反应发生。

Objective:To explore the effect of continuous suture and interrupted suture of perineal lateral incision of women,and to study their influence on the recovery of the incision of the women.Methods:141 women with perineal lateral incision after natural delivery were selected as the research subjects and were divided into two groups by random number table method between March 2019 and March 2021.71 women in group A were given continuous suture of perineal lateral incision,and 70 women in group B were given interrupted suture of perineal lateral incision.The women in the two groups were followed up for 3 months.The time differences of incision suture,healing,incision pain,and hospital stay of the women were compared between the two groups.The incision healing situations of the women in the two groups after 3 days of delivery were analyzed.The sexual function of the women in the two groups in the 3th month after delivery were evaluated by Female Sexual Function Index(FSFD),and the incidences of the adverse reactions in incision of the women during hospitalization and follow-up were compared between the two groups.Results:The perineal incision suture time(15.5±2.7 min),the healing time(5.9±2.0 d),the incision pain time(2.3±0.6 d),and the time of hospital stay(5.8±1.1 d)of the women in group A were all significantly shorter than those(20.1±2.6.min,7.3±1.9 d,3.0±0.7 d,and 6.8±1.6d)of the women in group B.The wound healing quality of the women in group A in the 3rd day after delivery was significantly better than that of the women in group B.The scores of FSFI of the women in group A in the postpartum 3rd month were significantly higher than those of the women in group B(all P<0.05).There were no significant differences in the incidence of the line reaction in incision,and the scar tenderness and infection of the women between the two groups(P>0.05).The incidence of subcutaneous scleroma(2.8%)of the women in group A was significantly lower than that(11.4%)of the women in group B(P<0.05).Conclusion:The operation time of continuous suture of perineal lateral incision of the women is shorter than that of interrupted suture,which is beneficial to the incision healing and the maternal sexual function,with less adverse reactions after suture.
