摘要
目的探索长期多次捐献机采血小板对成人骨密度的影响,为长期多次捐献机采血小板者的骨密度是否受到影响提供依据。方法采用病例对照设计,随机从2016年1月至2021年12月在韶关市中心血站无偿献血队伍中(且资料完整的捐献者)挑选93名。将骨密度降低的25例作为病例组,另外68名作为对照组。收集他们的年龄、性别和血浆钙(Ca)、磷(P)、25-羟维生素D_(3)[25-(OH)D_(3)]、甲状旁腺激素(PTH)以及骨密度(BMD)值,采用多重线性回归和多分类Logistic回归分析长期多次捐献机采血小板者血清Ca、P、25-(OH)D_(3)、PTH与BMD的相关性。结果以骨密度为因变量,分别以年龄、性别和血清Ca、P、25-(OH)D_(3)、PTH以及长期多次捐献机采血小板为自变量建立多重线性回归模型,年龄是骨密度减低的危险因素(P<0.5),血PTH和25-(OH)D_(3)则是保护因素,性别、血浆Ca、P和长期多次捐献机采血小板不是骨密度减低的危险因素。Logistic回归分析也显示年龄是危险因素,而血浆PTH是骨密度减低的保护因素(P<0.05),长期多次捐献机采血小板不是骨密度减低的危险因素。结论长期多次捐献机采血小板不是骨密度降低的危险因素,不会引起骨密度降低。
Objective To explore the effect of long-term multiple platelet donation on bone mineral density(BMD)of adults,and to provide evidence for whether BMD of long-term multiple platelet donation recipients is affected.Methods A case-control design was used to randomly select 93 blood donors(with complete data)from the unpaid blood donation team in Shaoguan Central Blood Station from January 2016 to December 2021.The 25 persons with decreased bone density were taken as the case group,and the other 68 persons were taken as the control group.Their age,sex and plasma calcium(Ca),phosphorus(P),25 hydroxyvitamin D_(3)[25-(OH)D_(3)],parathyroid hormone(PTH)and bone mineral density(BMD)values were collected,and the correlation between serum Ca,P,25-(OH)D_(3),PTH and BMD was analyzed by multiple linear regression and multiple logistic regression.Results A multiple linear regression model was established with BMD as dependent variable,and age,sex,serum Ca,P,25-(OH)D_(3),PTH,and long-term platelet donation as independent variables.Age was a risk factor for BMD reduction(P<0.05),blood PTH and 25-(OH)D_(3) were protective factors,and sex,plasma Ca,P,and long-term platelet donation were not risk factors for BMD reduction.Logistic regression analysis also showed that age was a risk factor,while plasma PTH was a protective factor for bone mineral density reduction(P<0.05),and longterm multiple platelet donation was not a risk factor for bone mineral density reduction.Conclusion Long term multiple donor platelet collection is not a risk factor for bone mineral density reduction,and will not cause bone mineral density reduction.
