摘要
目的 观察太极拳对慢性疲劳综合征(CFS)患者的疗效及对功能性磁共振成像中分数低频振幅(fALFF)的特征性影响。方法 招募CFS患者20例设为试验组,并选择性别、年龄与试验组差异无统计学意义的健康受试者20例设为对照组。两组受试者均接受为期4周的二十四式太极拳训练。在练习太极拳前后进行疲劳量表14 (FS-14)、生活质量量表(SF-36)及匹兹堡睡眠质量指数问卷(PSQI)评分;并进行脑部功能性磁共振成像检测,分析CFS患者和健康受试者fALFF值的差异,以及两者练习太极拳前后脑fALFF值的变化和差异,并对试验组受试者练习太极拳前后FS-14量表差值与脑fALFF差值进行相关性分析。结果练习前试验组FS-14评分、PSQI评分明显高于对照组,SF-36评分明显低于对照组(P<0.01)。与练习前比较,试验组练习后FS-14评分、PSQI评分均降低,SF-36评分升高(P<0.01);而对照组练习前后FS-14评分、PSQI评分差异无统计学意义(P>0.05),练习后SF-36评分较练习前显著升高(P<0.05)。与练习前比较,试验组受试者练习太极拳后fALFF值显著减弱的脑区包括左侧眶回/额极皮层、右侧前扣带回、右外侧颞叶,fALFF值显著增强的脑区包括左侧感觉运动皮层、右侧视觉皮层。试验组受试者练习前后全脑fALFF差值与FS-14评分差值呈负相关(r=-0.842,P<0.001)。结论 太极拳运动可改善CFS患者疲劳状态和睡眠,提高生活质量,可能与对患者脑部感觉运动皮层、左侧额叶、右外侧颞叶、前扣带回、视觉皮层的fALFF具有特异性影响有关。
Objective To explore the effect of Tai Chi on fractional amplitude of low-frequency fluctation(fALFF) in patients with chronic fatigue syndrome(CFS) by functional magnetic resonance.Methods Twenty subjects with CFS and 20 healthy subjects were recruited as observation group and control group,with no significant difference in gender and age.Both groups took 24 Form Tai Chi exercise for 4 weeks.The Fatigue Scale 14(FS-14),the 36-Item Short Form Survey(SF-36) and the Pittsburgh Sleep Quality Index Questionnaire(PSQI) were scored before and after practicing Tai Chi.Magnetic resonance scanning was performed to analyze the difference of fALFF between CFS patients and healthy subjects,as well as the change and difference of fALFF before and after Tai Chi training.In addition,the correlation between FS-14 change before and after Tai Chi and fALFF change in CFS patients was analyzed.Results At baseline,compared with healthy controls,the CFS subjects had higher FS-14 score and PSQI score,as well as lower SF-36 score(P<0.01).After Tai Chi practice,the FS-14 score,PSQI score were remarking reduced and SF-36 score significantly increased in the CFS patients(P<0.01),while there was no significant difference of FS-14 score and PSQI score in healthy controls(P>0.05) except for the increased SF-36 score(P<0.05).After Tai Chi practice,the fALFF of CFS subjects showed significant decrease in specific brain regions including left orbital gyrus/frontopolar cortex,right anterior cingulate,and right lateral temporal lobe,as well as increase in the regions of left sensorimotor cortex and right visual cortex.Moreover,the change of fALFF before and after Tai Chi training in the subjects with CFS was negatively correlated with the change of FS-14 scores(r=-0.842,P<0.001).Conclusion Tai Chi exercise can improve the fatigue and sleep condition and the quality of life in people with CFS,which may be related to its specific effect on fALFF of the sensorimotor cortex,left frontal cortex,right lateral temporal lobe,anterior cingulate gyrus,and visual cortex.
