移动网络媒介与医患信任危机——基于居民风险感知视角的实证分析

Mobile Network Media and Doctor-patient Trust Crisis——An Epirical Analysis Based on Residents’Risk Perception Perspective

良好的医患关系是维持医疗系统可持续发展和维护社会和谐稳定的重要基础。但随着互联网的不断发展,移动网络媒介放大了对不良医患事件的影响,给全社会带来严重的信任危机。文章采用2018年中国家庭追踪调查(CFPS)数据,考察了移动网络媒介对居民医患信任的影响,并探讨了该影响在不同群体中可能存在的异质性及其影响机制。研究结果表明,移动网络媒介显著降低了居民的医患信任水平,这一结果在利用倾向得分匹配模型(PSM)消除样本选择性偏差后依然保持稳健。异质性分析结果表明,移动网络媒介对居民医患信任的影响在不同性别和不同教育水平方面存在显著的异质性。相比于女性,移动网络媒介对男性居民医患信任水平的负面影响更为显著。此外,移动网络媒介的使用可以显著降低未受过高等教育居民的医患信任水平,而对于受过高等教育的居民则具有显著的正向促进作用。通过进一步中介分析发现,风险感知是移动网络媒介与居民医患信任之间的重要中介渠道,具体表现为移动网络媒介加剧了居民的风险感知水平,进而降低了居民的医患信任。今后相关部门应加强对网络环境的监管,营造安全健康的网络环境;加强舆论监管,强化积极的舆论导向;提升公众数字网络素养,加强网络信息鉴别能力。

A good doctor-patient relationship is an important foundation for maintaining sustainable development of the healthcare system and maintaining social harmony and stability.However,with the continuous development of the Internet,mobile network media have amplified the impact on adverse doctor-patient events,bringing a serious trust crisis to the whole society.Using data from the 2018 China Family Panel Studies(CFPS),the article examines the impact of mobile network media on residents’doctor-patient trust and explores the possible heterogeneity of this impact among different groups and its impact mechanism.The results showed that mobile web media significantly reduced residents’doctor-patient trust,and this result remained robust after eliminating sample selectivity bias using a propensity score matching model(PSM).The results of the heterogeneity analysis showed that there was significant heterogeneity in the effect of mobile web media on residents’doctor-patient trust by gender and education level.The negative impact of mobile web media on residents’doctor-patient trust level was more significant for men than for women.In addition,the use of mobile web media significantly reduced the level of doctor-patient trust of residents with no higher education,while it had a significant positive contribution for residents with higher education.Further mediation analysis revealed that risk perception is an important mediation channel between mobile network media and residents’doctor-patient trust,which is manifested by the fact that mobile network media exacerbates residents’risk perception level and thus reduces their doctor-patient trust.In the future,relevant departments should strengthen the regulation of the online environment to create a safe and healthy online environment;strengthen the regulation of public opinion to reinforce positive public opinion guidance;enhance public digital network literacy and strengthen the ability to discriminate online information.
