肌筋膜松解术联合本体感觉神经肌肉促进术治疗髌股疼痛综合征32例临床观察

Clinical observation on 32 cases of patellofemoral pain syndrome treated with myofascial release combined with PNF training

目的 观察肌筋膜松解术联合本体感觉神经肌肉促进术(PNF)治疗髌股疼痛综合征(PFPS)的临床疗效。方法 将64例PFPS患者采用随机数字表法分为对照组和治疗组,各32例。对照组采用超短波联合PNF治疗,治疗组采用肌筋膜松解术联合PNF治疗。2组均每周治疗4次,连续治疗6周。分别于治疗前及治疗6周、随访1个月后比较2组的视觉模拟评分法(VAS)评分、膝关节Lysholm功能评分以及臀大肌、股内侧肌、股外侧肌的表面肌电中位频率(MF)值。结果 治疗6周及随访1个月后2组VAS评分明显降低,Lysholm膝关节功能评分,臀大肌、股内侧肌、股外侧肌的表面肌电MF值明显升高,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组改善更明显,与对照组治疗后比较差异均有统计学意义(P<0.05)。结论 肌筋膜松解术联合PNF可减轻PFPS患者的疼痛程度,改善膝关节功能,且短期内效果维持佳。

Objective To observe the clinical efficacy of myofascial release combined with proprioceptive neuromuscular facilitation(PNF) in the treatment of patellofemoral pain syndrome(PFPS).Methods 64 patients with PFPS were randomly divided into a control group and a treatment group, with 32 patients in each group.The control group was treated with ultrashort wave combined with PNF,while the treatment group was treated with myofascial release combined with PNF.Both groups were treated 4 times a week for 6 consecutive weeks.The visual analogue scale(VAS)score, the knee joint Lysholm function score, and the surface electromyographic median frequency(MF) values of the gluteus maximus, medial femoral, and lateral femoral muscles were compared between the two groups before treatment, 6 weeks after treatment, and 1 month of follow-up.Results After 6 weeks of treatment and 1 month of follow-up, the VAS score of the two groups significantly decreased, while the Lysholm knee function score, the MF values of the gluteus maximus, medial femoral muscle, and lateral femoral muscle significantly increased, with a statistically significant difference compared to the same group before treatment(P<0.05),and the treatment group improved more significantly, with a statistically significant difference compared to the control group after treatment(P<0.05).Conclusion Myofascial release combined with PNF can alleviate pain in patients with PFPS,improve knee function, and maintain a good short-term effect.
