援土固胎汤联合地屈孕酮治疗早期妊娠先兆流产的临床疗效

Clinical effect of Yuantu Gutai decoction combined with didroxyprogesterone in the treatment of threatened abortion in early pregnancy

目的探讨援土固胎汤联合地屈孕酮治疗早期妊娠先兆流产临床疗效。方法选取2020年1—12月本院诊治的92例早期妊娠先兆流产患者作为研究对象,按照抽签法分为参照组和联合组,每组46例。参照组口服地屈孕酮治疗,联合组口服援土固胎汤联合地屈孕酮治疗,比较两组生殖激素水平(孕酮、人绒毛膜促性腺激素)、临床症状评分及临床疗效。结果治疗后,两组孕酮、人绒毛膜促性腺激素水平均高于治疗前,且联合组高于参照组,差异有统计学意义(P<0.05)。治疗后,两组腹部疼痛坠胀、腰部酸痛、阴道出血量和出血时间评分均低于治疗前,且联合组低于参照组,差异有统计学意义(P<0.05)。联合组治疗总有效率为95.65%,高于参照组的76.09%,差异有统计学意义(P<0.05)。结论援土固胎汤联合地屈孕酮治疗早期妊娠先兆流产疗效显著,可改善生殖激素水平,缓解或消除临床症状,值得临床推广应用。

Objective To explore the clinical effect of Yuantu Gutai decoction combined with didroprogesterone in the treatment of threatened abortion in early pregnancy.Methods 92 patients with threatened abortion in early pregnancy diagnosed and treated in our hospital from January to December 2020 were selected as the research subjects,and they were divided into the reference group and the combination group according to the lottery method,with 46 cases in each group.The reference group was treated with dydrogesterone orally,and the combination group was treated with dydrogesterone combined with Yuantu Gutai decoction.The levels of reproductive hormone(progesterone,human chorionic gonadotropin),clinical symptom score and clinical efficacy were compared between the two groups.Results After treatment,the levels of progesterone and human chorionic gonadotropin in the two groups were higher than before treatment,and the combination group was higher than the reference group,the differences were statistically significant(P<0.05).After treatment,abdominal pain and distension,waist soreness,vaginal bleeding volume and bleeding time scores in the two groups were lower than before treatment,and the combination group was lower than the reference group,the differences were statistically significant(P<0.05).The total effective rate of the combination group was 95.65%,which was higher than 76.09%of the reference group,and the difference was statistically significant(P<0.05).Conclusion The effect of Yuantu Gutai decoction combined with dydrogesterone in the treatment of early pregnancy threatened abortion is significant,which can improve the level of reproductive hormones,relieve or eliminate clinical symptoms,and is worthy of clinical promote and application.
