县域医共体处方干预同质化管理体系构建及效果分析

Construction and effect analysis of homogeneous management system for prescription intervention in county medical community

目的 构建县域医共体质量管理平台为基础的处方干预同质化管理体系并对管理效果进行评价。方法 以《医院处方点评管理规范(试行)》《医疗机构处方审核规范》为指导,制定同质化的处方干预标准,采用winning 2019审方软件和药师人工干预相结合的方式,通过建立医共体处方干预集成平台并依托医共体质量管理中心,使不合理处方药师干预和行政干预双管齐下,构建出一套科学、完善、具有操作性的处方干预审核管理体系。采取回顾性分析,提取2022年1—4月处方分别为168 512张、140 125张、172 691张和168 463张,汇总分析winning2019审方软件干预和药师人工干预记录,应用Excel 2017软件进行分类计算整理及评价。结果 通过县域医共体质量管理平台的处方干预同质化管理体系构建,使医共体不合理处方监测面提升,干预执行力度增强,处方合格率逐月提高,2022年1—4月分别为81.12%、91.69%、96.99%和98.29%结论 通过基于县域医共体质量管理平台的处方干预同质化管理体系构建,能够提高医共体处方合格率,从而规范医疗行为,促进临床合理用药,保障医疗安全。

Objective To construct a homogeneous management system for prescription intervention based on the quality management platform of county medical community and to evaluate the management effect.Methods The Hospital Prescription Review Management Standards(Trial)and Medical Institution Prescription Review Specificationswere used as the guide to develop homogenized prescribing intervention criteria.A scientific,perfect and operational prescription intervention audit management systemwas built by using a combination of Win-ning 2019 prescription software and pharmacist manual intervention,that is,establishing the medical community prescription intervention integration platform and relying on the medical community quality management center to jointly adoptpharmacist intervention and administrative intervention of irrational prescription.A retrospective analy-sis was madeby extracting 168512,140125,172691 and 168463 prescriptions from January 2022 to April 2022,collecting and analyzing the records of Winning2019 prescription software intervention and pharmacist manu-al intervention,and summarizing and evaluating with Excel2017 software.Results Through the construction of the homogeneous management system of prescription intervention on the quality management platform of the county medical community,the monitoring coverage of irrational prescriptions in the medical community was improved,the intervention implementation was enhanced,and the prescription qualification rate increased month by month,which was 81.12%,91.69%,96.99%and 98.29%respectively from January to April 2022.Conclusions The construction of the homogeneous management system of prescription intervention based on the quality management platform of the county medical community can improve the prescription qualification rate of the medical community,so as to standardize medical behavior,promote the rational clinical use of drugs,and ensure medical safety.
