某三级综合医院非计划再次手术情况回顾性分析

Retrospective Analysis of Unplanned Reoperations in A Third-tier General Hospital

目的分析某三级综合医院非计划再次手术发生情况,探索降低非计划再次手术发生率,提高医疗质量安全的有效措施及管理方法。方法收集某三级综合医院2021年1月1日—2022年12月31日235例非计划再次手术的相关数据,对发生率、科室分布、再次手术发生原因等进行统计分析。结果2022年全院非计划再次手术发生率0.194%,低于2021年的0.212%;2022年非计划再次手术原因占比为:术后出血56.60%,术后感染8.94%,出现漏或瘘5.96%,术后粘连、梗阻5.11%,术后脑水肿、脑疝4.68%,术后血管再次闭塞3.40%。结论医院必须加强非计划再次手术监管,重视原因分析,针对存在的问题进行持续改进,降低非计划再次手术的发生率,提高医疗安全质量。

Objective To analyze the occurrence of unplanned reoperation in a tertiary general hospital,and explore effective measures and management methods to reduce the incidence of unplanned reoperation and improve medical quality and safety.Methods By collecting data related to 235 cases of unplanned reoperation in a tertiary general hospital from January 1,2021,to December 31,2022,the incidence,departmental distribution,and causes of reoperation occurrence were statistical analyzed.Results In 2022,the incidence of unplanned reoperation in hospital was 0.194%,which was lower than 0.212%in 2021.The percentage of unplanned reoperation causes in 2022 was 56.60%for postoperative bleeding,8.94%for postoperative infection,5.96%for the presence of a leak or fistula,5.11%for postoperative adhesions and obstruction,4.68%for postoperative cerebral edema and brain herniation,and 3.40%for postoperative vascular re-occlusion.Conclusion The hospital must strengthen the supervision of unplanned reoperation,pay attention to the analysis of the causes,carry out continuous improvement in view of the existing problems,reduce the incidence of unplanned reoperation,and improve the quality of medical safety.
