腹腔镜辅助胃癌根治术治疗进展期胃癌的效果及对术后炎症指标的影响

Effect of Laparoscopic-assisted Radical Gastrectomy on Advanced Gastric Cancer and Its Influence on Postoperative Inflammatory Indexes

目的:分析腹腔镜辅助胃癌根治术(laparoscopic-assisted radical gastrectomy,LARG)治疗进展期胃癌(advanced gastric cancer,AGC)的效果及对术后炎症指标的影响。方法:选取丰城市人民医院2020年5月—2022年8月收治的68例AGC患者为本次研究对象,并以随机数字表法分为常规组(34例,开腹胃癌根治术治疗)和观察组(34例,LARG治疗),对比分析两组手术方案的应用价值。结果:两组术前的炎症指标、胃功能指标及淋巴结清扫数量比较,差异均无统计学意义(P>0.05)。观察组切口长度短于常规组,术中失血量少于常规组,术后并发症发生率、术后炎症指标及胃功能指标均显著低于常规组(P<0.05)。观察组术后排气时间、卧床时间、恢复正常饮食时间、住院时间均显著短于常规组(P<0.05)。观察组完全缓解率显著高于常规组(P<0.05)。观察组手术时长显著长于常规组(P<0.05)。结论:LARG治疗AGC患者,可取得理想的手术效果。

Objective:To analyze the effects of laparoscopic-assisted radical gastrectomy(LARG)for the treatment of advanced gastric cancer(AGC)and its impacts on postoperative inflammatory indicators.Method:A total of 68 patients with AGC who were admitted to Fengcheng People's Hospital from May 2020 to August 2022 were selected as the subjects of this study.They were divided into the conventional group(34 patients,with laparotomy for gastric cancer)and the observation group(34 patients,with LARG treatment)by random number table method.The application value of the two surgical schemes was compared and analyzed.Result:There were no significant differences between the two groups in the preoperative inflammatory index,gastric function index and the number of lymph node dissection(P>0.05).The incision length in the observation group was shorter than that in the conventional group,intraoperative blood loss was less than that of the conventional group,postoperative complication rate,postoperative inflammatory indexes and gastric function indexes in the observation group were significantly lower than those in the conventional group(P<0.05).Postoperative exhaust time,bed rest time,normal diet time and hospital stay time in the observation group were significantly shorter than those in the conventional group(P<0.05).The complete response rate in observation group was significantly higher than that in conventional group(P<0.05).The operation time in the observation group was significantly longer than that in conventional group(P<0.05).Conclusion:LARG can achieve ideal results in the treatment of AGC patients.
