河南省既往有偿献血人群慢性丙型肝炎重度肝纤维化的影响因素分析

Influencing factors of severe hepatic fibrosis caused by chronic hepatitis C in previously paid blood donors in Henan Province

目的探讨河南省既往有偿献血人群慢性丙型肝炎重度肝纤维化预后的影响因素,为未来临床鉴别重度肝纤维化高危人群提供循证依据。方法抽取2019年3月至2021年5月河南省既往有偿献血人群200例,样本均确诊为慢性丙型肝炎,其中重度肝纤维化患者83例纳入观察组,117例非重度肝纤维化患者纳入对照组。采用单因素和多因素Logistic分析慢性丙型肝炎患者重度肝纤维化预后的影响因素。结果两组患者病程、长期饮酒、糖尿病、白细胞计数、抗病毒治疗五项指标比较,差异有统计学意义(P<0.05)。多因素Logistic分析结果显示,病程≥2年、长期饮酒、糖尿病、白细胞异常、未接受抗病毒治疗为影响慢性丙型肝炎重度肝纤维化进展的重要因素(P<0.05)。结论血站需警惕因献血导致的丙型肝炎病毒血液传播问题;针对慢性丙型肝炎患者,若其病程≥2年、长期饮酒、糖尿病、白细胞异常、未接受抗病毒治疗,需要加大对其的重度肝纤维化预后关注。

ObjectiveTo investigate the influencing factors of the prognosis of severe hepatic fibrosis caused by chronic hepatitis C in previously paid blood donors in Henan Province,and provide evidence-based reference for identifying high-risk groups of severe hepatic fibrosis in future clinical practice.MethodsTwo hundred previously paid blood donors in Henan Province from March 2019 to May 2021 were selected.All samples from enrolled subjects were confirmed as chronic hepatitis C.Among them,83 patients with severe hepatic fibrosis were included in the observation group,and 117 patients without severe hepatic fibrosis were included in the control group.The influencing factors of prognosis of severe hepatic fibrosis in patients with chronic hepatitis C were analyzed by univariate and multivariate logistic analysis.ResultsThere were significant differences between the two groups in the course of disease,long-term drinking,diabetes,white blood cell count,and antiviral treatment(P<0.05).The results of multivariate logistic analysis showed that,the course of disease≥2 years,long-term drinking,diabetes,abnormal white blood cell count,and failure to receive antiviral treatment were important factors affecting the progress of severe hepatic fibrosis in patients with chronic hepatitis C(P<0.05).ConclusionsBlood stations should be alert to the problem of transmission of blood infected with hepatitis C virus during blood donation.For chronic hepatitis C patients with disease course more than 2 years,long-term drinking,diabetes,abnormal white blood cellcount,and no antiviral treatment,more attention should be paid to the prognosis of severe hepatic fibrosis.
