应用鼻内镜鼻窦手术联合低温等离子下鼻甲射频消融术对治疗鼻窦炎的疗效分析 被引量:2

摘要 目的:分析鼻内镜鼻窦手术联合低温等离子下鼻甲射频消融术对治疗鼻窦炎的疗效。方法:选取2019年10月—2021年6月河南科技大学第二附属医院收治的84例鼻窦炎患者作为研究对象,采取随机数表法将其分为研究组(42例)与对照组(42例)。对照组患者行鼻内镜鼻窦手术治疗,研究组患者在对照组的基础上加用低温等离子下鼻甲射频消融术治疗,观察两种治疗方案的应用效果,为该类患者的临床治疗提供科学依据。结果:两组患者临床疗效情况比较,研究组患者优于对照组,差异有统计学意义(χ^(2)=5.486,P<0.05);术后3个月,两组患者各项临床症状评分比较,研究组患者各项评分更低,差异有统计学意义(t=8.534、6.048、6.915、5.061、7.357,P<0.05);两组患者鼻腔黏液纤毛清除能力、术后总鼻腔阻力,差异有统计学意义(t=14.055、10.786、19.193、4.359,P<0.05);术后3个月和术后6个月,研究组患者嗅觉功能均优于对照组,差异有统计学意义(χ^(2)=6.459、6.114,P<0.05);术后6 d,研究组患者IL-6、IL-8、CRP、TNF-α水平显著低于对照组,差异有统计学意义(t=2.138、29.409、3.362、8.042,P<0.05);两组患者术后并发症发生率相比,差异无统计学意义(χ^(2)=0.156,P>0.05)。结论:鼻内镜鼻窦手术联合低温等离子下鼻甲射频消融术的应用可显著改善鼻窦炎患者病情,此术式安全、高效。 Objective:To analyze the efficacy of nasal endoscopic sinus surgery combined with low-temperature plasma inferi⁃or turbinate radiofrequency ablation in the treatment of sinusitis.Methods:84 patients with rhinosinusitis admitted to the hospital from October 2019 to June 2021 were selected as the study population and were divided into study group(42 cases)and control group(42 cases)by random number table method.The latter was treated with nasal endoscopic sinus surgery,and the former was treated with radiofrequency ablation of inferior turbinate with cryo-plasma on top of the latter.The application effect of the two treatment protocols was observed to provide a scientific basis for the clinical treatment of this type of patients.Results:The clini⁃cal efficacy of the study group was better than that of the control group,and the difference was statistically significant(χ^(2)=5.486,P<0.05).At 3 months postoperatively,all clinical symptom scores were lower in the study group,with statistically significant dif⁃ferences(t=8.534,6.048,6.915,5.061,7.357,P<0.05).The differences in nasal mucus cilia clearance ability and total postopera⁃tive nasal resistance between the two groups were statistically significant(t=14.055,10.786,19.193,4.359,P<0.05).At 3 months and 6 months after surgery,patients in the study group had better olfactory function than the control group,and the difference was statistically significant(χ^(2)=6.459,6.114,P<0.05).At 6 d postoperatively,the levels of IL-6,IL-8,CRP,and TNF-αwere signifi⁃cantly lower in the study group than in the control group,with statistically significant differences(t=2.138,29.409,3.362,8.042,P<0.05).There was no statistically significant difference in the incidence of postoperative complications between the two groups compared with each other(χ^(2)=0.156,P>0.05).Conclusion:The application of nasal endoscopic sinus surgery combined with low-temperature plasma inferior turbinate radiofrequency ablation can significantly improve the condition of patients with rhinosi⁃nusitis,and this procedure is safe,efficient.
