抑郁症患者药物治疗依从性影响因素及预防对策

Related factors influencing compliance of drug therapy in depression patients and preventive strategies

目的 探讨影响抑郁症患者药物治疗依从性相关因素及预防对策。方法 2020年1月~2021年12月诊治158例抑郁症患者,根据治疗依从性评估结果对影响因素进行分析,包括年龄、性别、受教育年限、病程、抑郁症知识、职业、病耻感、婚姻、精神病家族史、体重指数、月收入水平、医疗费用、居住状况、药物不良反应。结果 158例抑郁症患者中依从性差50例(31.65%),依从性好108例(68.35%),抑郁症患者药物治疗依从性14个指标存在显著相关性(P <0.05),Logistic回归分析影响依从性因素:病耻感(OR=3.927,95%CI:1.043~14.962)、月收入低(OR=5.785,95%CI:1.298~24.765)、病程≥5年(OR=3.650,95%CI:0.987~17.875)、药物不良反应(OR=4.260,95%CI:1.085~18.368)。结论 影响抑郁症患者治疗依从性的因素众多,在其治疗期间及时根据相关影响因素开展针对性预防措施。

Objective To explore related factors influencing compliance of drug therapy in depression patients and preventive measures.Methods From January 2020 to December 2021,158 depression patients were diagnosed and treated.According to evaluation results of treatment compliance,the relevant factors influencing were analyzed,including age,gender,years of education,course of disease,depression knowledge,occupation,stigma,marriage,family history of psychosis,body mass index,monthly income level,medical expenses,living conditions,and adverse drug reactions.Results Of 158 depression patients,50 cases were with poor compliance(31.65%),108 cases were with good compliance(68.35%).There was significant correlation among 14 indicators of drug treatment compliance in patients with depression(P<0.05).Logistic regression analysis of factors affecting compliance were:Stigma(OR=3.927,95%CI:1.043~14.962),the monthly income is low(OR=5.785,95%CI:1.298~24.765,course of disease≥5 years(OR=3.650,95%CI:0.987~17.875),adverse drug reactions(OR=4.260,95%CI:1.085~18.368).Conclusion There are many factors that affect the treatment compliance of patients with depression.During the treatment,targeted preventive measures should be taken according to relevant factors.
