摘要
目的探究基于MRI影像学探讨股骨滑车面对称性、股骨滑车深度、髌骨移动率(BSO)与髌股关节不稳的相关性。方法回顾性分析选取该院2021年1月—2022年2月收治的35例双膝及髌骨关节不稳患者为观察组,选择同期体检的健康人群35例为对照组。两组患者采取仰卧位双膝完全伸直进行MRI检查。观察两组患者股骨滑车面对称性、股骨滑车深度、BSO与髌股关节不稳及股骨滑车与胫骨结节间的水平距离(TT-TG)。结果观察组TT-TG水平与对照组差异无统计学意义(P>0.05)。观察组股骨滑车面对称性、股骨滑车深度水平显著低于对照组,BSO水平显著高于对照组(P<0.05)。股骨滑车面对称性与髌股关节不稳呈现中度负相关(r=-0.518,P<0.01)。股骨滑车深度与髌股关节不稳呈现负相关(r=-0.492,P<0.01)。BSO与髌股关节不稳呈现正相关性(r=-0.562,P<0.01)。结论股骨滑车面对称性、股骨滑车深度与髌股关节不稳呈现反相关,BSO与髌股关节不稳呈现正相关。
Objective To investigate the correlation of femoral trochlear facet symmetry,trochlear depth,patellar lateral displacement rate with patellofemoral instability based on MRI.Methods Thirty-five patients with bilateral knee joint instability or patellofemoral instability in the hospital from January 2021 to February 2022 were set as observation group,meantime,another 35 healthy individuals were enrolled as control group.Patients received MRI examination with knee extension in supine position.Then the femoral trochlear facet symmetry,trochlear depth,patellar lateral displacement bisect-offset(BSO),patellofemoral instability,and tibial tubercle-trochlear groove(TT-TG)distance were recorded.Results TT-TG distance yielded no statistical difference between two groups(P>0.05).The femoral trochlear facet symmetry and trochlear depth of observation group were significantly lower than those of control group,and the BSO was significantly larger than that of control group,with statistical difference(P<0.05).The femoral trochlear facet symmetry showed a moderate negative correlation with patellofemoral instability(r=-0.518,P<0.01).Femoral trochlear depth showed a negative correlation with patellofemoral instability(r=-0.492,P<0.01).BSO showed a positive correlation with patellofemoral instability(r=-0.562,P<0.01).Conclusion Femoral trochlear facet symmetry and trochlear depth are negatively correlated with patellofemoral instability,and patellar lateral displacement is positively correlated with patellofemoral instability.
