厚朴排气合剂联合卡孕栓治疗对剖宫产术后患者GAS、MLT及VIP水平的影响

Effect of Houpu exhaust mixture combined with Kamei suppository on GAS,MLT and VIP levels of patients after cesarean section

目的分析厚朴排气合剂联合卡孕栓治疗对剖宫产术后患者GAS、MLT及VIP水平的影响。方法选取2018年3月至2019年6月于秦皇岛市第二医院行剖宫产术且无用药禁忌症的产妇180例,根据治疗方式不同分成A组(厚朴排气合剂联合卡孕栓治疗)60例、B组(厚朴排气合剂治疗)60例及C组(卡孕栓治疗)60例,对比三组术后恢复时间、住院时间及产后24 h出血量、体征评分、血清GAS、MLT及VIP水平及并发症发生情况。结果术后肠鸣音恢复时间、首次排气时间、首次排便时间、住院时间、产后24 h出血量:C组<B组<A组,差异均有统计学意义(P<0.05)。三组术后1 d体征评分比较,差异无统计学意义(P>0.05),体征评分术后2 d、术后3 d:C组<B组<A组,差异均有统计学意义(P<0.05)。VIP术后1 d、术后3 d水平:A组<B组<C组,GAS、MLT术后1 d、术后3 d水平:C组<B组<A组,差异具均有统计学意义(P<0.05)。A组并发症发生占比低于B组、C组,但三组间比较差异无统计学意义(P>0.05)。结论厚朴排气合剂联合卡孕栓治疗可促进剖宫产术后患者胃肠功能恢复,改善产妇临床症状和体征,其作用机制可能与升高患者血清GAS和MTL水平、抑制VIP水平有关。

Objective To analyze the effects of Houpu exhaust mixture combined with Kamei suppository on GAS,MLT and VIP levels of patients after cesarean section.Methods 180 cases of parturient who underwent cesarean section in the Second Hospital of Qinhuangdao City from March 2018 to June 2019 and had no drug contraindications were selected,and according to different treatment methods,they were divided into group A(Hopu exhaust mixture combined with card pregnancy suppository treatment)of 60 cases,group B(Houpu Paiqi mixture treatment)of 60 cases and group C(carprogesterone suppository treatment)of 60 cases,the postoperative recovery time,hospitalization time,postpartum 24 h bleeding volume,sign score,serum GAS,MLT and VIP level,and the incidence of complications were compared among the three groups.Results The postoperative bowel sound recovery time,first exhaust time,first defecation time,hospital stay,postpartum blood loss 24 h:group C<group B<group A,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the three groups in the score of physical signs 1 day after surgery(P>0.05),and the score of physical signs 2 and 3 days after surgery:group C<group B<group A,the difference was statistically significant(P<0.05).VIP level 1 d and 3 d after surgery:group A<group B<group C;GAS and MLT levels 1 d and 3 d after surgery:group C<group B<group A,the difference was statistically significant(P<0.05).The incidence of complications in group A was lower than that in group B and group C,but there was no statistical significance among the three groups(P>0.05).Conclusion Hopu exhaust mixture combined with Carprost suppository treatment can promote the recovery of gastrointestinal function of patients and improve clinical symptoms and signs of parturient after cesarean section,its mechanism of action may be related to the increase of serum GAS and MTL levels,and the inhibition of VIP levels.
