HFMEA管理模式在不孕症门诊宫腔镜患者安全管理中的应用分析

Application analysis of HFMEA management model in the safety management of hysteroscopy patients in infertility clinic

目的:分析医疗失效模式与效应分析(HFMEA)在不孕症门诊宫腔镜患者安全管理中的应用效果。方法:选取2021年1月至10月在南京医科大学第一附属医院门诊行宫腔镜手术1620例患者,采用常规门诊手术管理模式作为对照组;实验组基于HFMEA管理模式改进后选取2022年2月至12月在我院门诊行宫腔镜手术1631例患者,比较两组的不良事件发生率及改进前后护理人员专业知识掌握程度。结果:通过应用HFMEA管理方法,明显降低风险危急值(RPN),有效降低不良事件发生率,差异有统计学意义(χ^(2)=14.226,P<0.001);护理人员通过培训及流程改进,考核理论和操作成绩均高于改进前,差异有统计学意义(P<0.05)。结论:HFMEA管理方法可有效规范工作流程,预防门诊宫腔镜手术不良事件发生率,强化风险意识,可有效保障患者医疗安全,持续提高门诊宫腔镜手术室护理管理质量。

Objective:To explore the application effect of healthcare failure mode and effect analysis(HFMEA)in the safety management of hysteroscopy patients in infertility clinic.Methods:A total of 1620 cases from outpatient department undergoing hysteroscopic surgery from January to October 2021 were selected as control group and routine outpatient surgery management mode was used.1631 cases from outpatient department with hysteroscopic surgery from February to December 2022 based on the improved HFMEA management mode were selected as experimental group.The incidence of adverse events and the professional knowledge of nurses before and after the improvement of the two groups were compared.Results:By applying HFMEA management method,the risk priority number(RPN)was significantly decreased and the incidence of adverse events was remarkably reduced,the difference being statistically significant.Through training and process improvement,the theoretical and operational performance of nurses was better than that before the improvement,and the difference was statistically significant.Conclusion:HFMEA management approach can effectively standardize the work process,prevent the incidence of adverse events in outpatient hysteroscopic surgery,strengthen risk awareness,guarantee the medical safety of patients,and continuously improve the quality of nursing management in outpatient hysteroscopic operating room.
