扶正祛疣汤联合胸腺肽对尖锐湿疣患者血清炎症因子水平及复发率影响研究

Effect of Fuzheng Quyou Decoction(扶正祛疣汤)Combined with Thymosin on Serum Inflammatory Factors and Recurrence Rate in Patients with Condyloma Acuminatum

目的探究扶正祛疣汤联合胸腺肽对尖锐湿疣患者血清炎症因子水平及复发率影响。方法收集2020年8月—2022年8月医院皮肤科收治的尖锐湿疣患者94例,根据随机数字分配法,分为试验组与对照组,每组47例。所有受试者患处经消毒处理后均涂重组人干扰素α2b凝胶,对照组予以胸腺肽肠溶胶囊,试验组在对照组的基础上予以扶正祛疣汤,所有患者均连续治疗2个月。治疗结束后对比分析两组临床疗效、局部人乳头瘤病毒(human papillomavirus,HPV)病毒载量、疼痛评估、炎性因子指标、T淋巴细胞指标、不良反应及复发率。结果治疗后与对照组比较,试验组总有效率较高(P<0.05)。与治疗前比较,治疗后2局部HPV载量降低,数字评分法(numerical rating scale,NRS)评分升高;与对照组比较,试验组局部HPV载量较低,NRS评分较高(P<0.05)。与治疗前比较,治疗后两组白细胞介素-2(IL-2)、干扰素γ(interferonγ,IFN-γ)水平升高,白细胞介素-12(interleukin-12,IL-12)、白细胞介素-4(interleukin-4,IL-4)、Th1/Th2水平均降低;与对照组比较,试验组IL-2、IFN-γ水平较高,IL-12、IL-4、Th1/Th2水平较低(P<0.05)。与治疗前比较,治疗后两组CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平升高,CD_(8)^(+)水平降低;与对照组比较,试验组CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平较高,CD_(8)^(+)水平较低(P<0.05)。对照组与试验组不良反应发生率相比无统计学意义(P>0.05)。对照组的3个月内的复发率为34.04%(16/47),试验组3个月内的复发率为6.38%(3/47),与对照组比较,试验组3个月内复发率较低(P<0.05)。结论对尖锐湿疣患者采取扶正祛疣汤联合胸腺肽,能够显著提高临床疗效,使炎症因子水平及T淋巴细胞亚群得到改善水平,提高免疫功能,安全性较高,具有临床推广价值。

Objective To explore the effect of Fuzheng Quyou Decoction(扶正祛疣汤)combined with thymosin on serum inflammatory factors and recurrence rate in patients with condyloma acuminatum.Methods A total of 94 patients with condyloma acuminatum admitted to the Department of Dermatology of the hospital from August 2020 to August 2022 were collected and divided into experimental group and control group according to the random number allocation method,with 47 cases in each group.All subjects were coated with recombinant human interferonα2b gel after disinfection treatment.The control group was given thymosin enteric-soluble capsule,and the experimental group was given Fuzheng Quyu Decoction on the basis of the control group.All patients were treated for 2 months.After treatment,the clinical efficacy,local human papillomavirus(HPV)viral load,pain assessment,inflammatory factors index,T lymphocyte index,adverse reactions and recurrence rate were compared between the two groups.Results Compared with that of the control group,the total effective rate of the experimental group was higher(P<0.05).Compared with those before treatment,the load of local HPV decreased and the numerical rating scale(NRS)score increased after treatment.Compared with the control group,the experimental group had lower local HPV load and higher NRS score(P<0.05).Compared with those before treatment,the levels of interleukin-2(IL-2)and interferonγ(IFN-γ)were increased,and the levels of interleukin-12(IL-12),interleukin-4(IL-4)and Th1/Th2 were decreased in the two groups after treatment.Compared with those of the control group,the levels of IL-2 and IFN-γin the experimental group were higher,and the levels of IL-12,IL-4 and Th1/Th2 were lower(P<0.05).Compared with those before treatment,the levels of CD_(3)^(+),CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) in the two groups increased after treatment,while the level of CD_(8)^(+) decreased.Compared with those of the control group,the levels of CD_(3)^(+),CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) in the experimental group were higher,and the level of CD_(8)^(+) was lower(P<0.05).There was no significant difference in the incidence of adverse reactions between the control group and the experimental group(P>0.05).The recurrence rate within 3 months in the control group was 34.04%(16/47),and that in the experimental group was 6.38%(3/47).Compared with that of the control group,the recurrence rate within 3 months in the experimental group was lower(P<0.05).Conclusion Fuzheng Quyou Decoction combined with thymosin for patients with condyloma acuminatum can significantly improve the clinical efficacy,the levels of inflammatory factors and T lymphocyte subsets and the immune function,and it has high safety and clinical promotion value.
