5-氨基酮戊酸光动力疗法联合二氧化碳激光治疗尖锐湿疣临床疗效分析

Clinical Effect of 5-aminolevulinic Acid Photodynamic Therapy Combined with Carbon Dioxide Laser in Treatment of Condyloma Acuminatum

目的 探究5-氨基酮戊酸光动力疗法(5-aminolevulinic acid photodynamic therapy, ALA-PDT)联合二氧化碳(carbon dioxide, CO_(2))激光与单纯光动疗法(photodynamic therapy, PDT)治疗尖锐湿疣(condyloma acuminatum, CA)临床疗效。方法 回顾性选取2021年1月—2022年12月漳浦县医院皮肤科收治的64例CA患者的临床资料,根据临床治疗方案分为单纯组(32例,单纯光动疗法治疗)与联合组(32例,5-氨基酮戊酸光动力疗法联合二氧化碳激光)。比较两组基线资料、临床疗效、人乳头瘤病毒(human papilloma virus, HPV)改善情况、复发情况、不良反应情况。结果 联合组临床治疗总有效率为93.75%、HPV转阴率为87.50%、明显高于单纯组的75.00%、62.50%,差异有统计学意义(χ^(2)=4.267、5.333,P<0.05)。联合组的疾病复发率12.50%,明显低于单纯组,差异有统计学意义(χ^(2)=5.333,P<0.05)。联合组HPV病毒载量下降率、不良反应发生率均优于对照组,但差异无统计学意义(P>0.05)。结论 CA临床治疗过程中,ALA-PDT联合CO_(2)激光治疗方案的临床应用效果较好,在患者治愈率、HPV转阴率提升,复发率减少等方面具有促进作用。

Objective To explore the clinical efficacy of 5-aminolevulinic acid photodynamic therapy(ALA-PDT)combined with carbon dioxide(CO_(2))laser and simple photodynamic therapy(PDT)in the treatment of condyloma acu-minatum(CA).Methods The clinical data of 64 CA patients admitted to the dermatology department of Zhangpu County Hospital from January 2021 to December 2022 were retrospectively analyzed.According to the clinical treat-ment plan,the patients were divided into a simple group(32 cases,photokinetic therapy alone)and a combined group(32 cases,5-amino-ketovalerate photodynamic therapy combined with carbon dioxide laser).Baseline data,clinical ef-ficacy,human papilloma virus(HPV)improvement,recurrence and adverse reactions were compared between the two groups.Results The total effective rate and HPV negative conversion rate in combination group were 93.75%and 87.50%,which were significantly higher than 75.00%and 62.50%in single group,and the difference was statistically significant(χ^(2)=4.267,5.333,P<0.05).The recurrence rate of combined group was 12.50%lower than that of simple group,and the difference was statistically significant(χ^(2)=5.333,P<0.05).The decrease rate of HPV viral load and in-cidence of adverse reactions in combination group were better than those in control group,but and the difference was not statistically significant(P>0.05).Conclusion In the clinical treatment process of CA,ALA-PDT combined with CO_(2) laser therapy has a good clinical application effect,which can promote the cure rate of patients,the increase of HPV negative conversion rate,and the reduction of recurrence rate.
