总前列腺特异性抗原、游离前列腺特异性抗原及癌胚抗原单独及联合检测对前列腺癌的诊断价值

Diagnostic value of total-prostate specific antigen,free-prostate specific antigen,and carcinoembryonic antigen single and combined detection for prostate cancer

目的 探讨总前列腺特异性抗原(T-PSA)、游离前列腺特异性抗原(F-PSA)及癌胚抗原(CEA)单独及联合检测对前列腺癌的诊断价值。方法 选取175例前列腺疾病患者,比较前列腺癌和前列腺良性疾病患者血清T-PSA、F-PSA、CEA水平。以病理检查为金标准,评估T-PSA、F-PSA、CEA单独及联合检测对前列腺癌的诊断价值。结果 病理检查结果显示,175例前列腺疾病患者中,前列腺癌61例,前列腺良性疾病患者114例。前列腺癌患者血清T-PSA、F-PSA、CEA的阳性表达率均明显高于前列腺良性疾病患者,差异均有统计学意义(P﹤0.01)。T-PSA+F-PSA+CEA检测诊断前列腺癌的灵敏度、特异度、准确度均高于三者单独检测(P﹤0.05)。结论T-PSA、F-PSA、CEA联合检测在前列腺癌患者血清中的阳性表达率均较高,三者联合检测对前列腺癌的诊断价值较高。

Objective To analyze the diagnostic value of total-prostate specific antigen(T-PSA),free-prostate specific antigen(F-PSA),and carcinoembryonic antigen(CEA) single and combined detection for prostate cancer.Method Selecting 175 patients with prostate disease,serum levels of T-PSA,F-PSA,and CEA were measured and compared in patients with prostate cancer and benign prostatic diseases.Using pathological examination as the gold standard,evaluate the diagnostic value of T-PSA,F-PSA,CEA single and combined detection for prostate cancer.Result Pathological examination showed that of the 175 patients with prostate disease,there were 61 cases of prostate cancer and 114 cases of benign prostate diseases.The positive expression rate of serum T-PSA,F-PSA and CEA in patients with prostate cancer were significantly higher than those in patients with benign prostate diseases(P<0.01).The sensitivity,specificity and accuracy of T-PSA + F-PSA + CEA in the diagnosis of prostate cancer were higher than those of single detection(P<0.05).Conclusion The positive expression rate of T-PSA,F-PSA and CEA in the serum of prostate cancer patients are higher,and the combined detection has higher diagnostic value for prostate cancer.
