形神共调针刺法配合早期康复训练对改善脑卒中后肢体运动功能的影响

Effect of acupuncture approach of coordinating the body and spirit combined with an early rehabilitation on improving limb movement at post-stroke

目的 观察形神共调针刺法配合早期康复训练对改善脑卒中后肢体运动功能的影响。方法 本研究为前瞻性研究,选取黄河三门峡医院2021年5月至2023年5月收治的110例脑卒中患者为研究对象,以计算机随机分组法将其分为常规组和联合组,各55例。常规组实施常规治疗+早期康复训练,联合组在常规组基础上实施形神共调针刺辅助治疗,比较两组患者的治疗效果。结果 联合组治疗后的脑电图(EEG)α波频率、β波频率、θ波频率均低于常规组;联合组治疗后的神经元特异性烯醇化酶(NSE)、中枢神经特异蛋白(S100β)均低于常规组;联合组治疗后的神经功能缺损量表(NIHSS)评分低于常规组,改良Barthel指数(MBI)评分高于常规组;联合组的简化Fugl-Meyer运动功能评定量表(FMA)上肢功能、下肢功能评分均高于常规组;随访半年后联合组不良事件发生率7.27%(4/55)低于常规组21.82%(12/55),P<0.05。结论 形神共调针刺法配合早期康复训练有利于促进脑卒中患者的脑电生理活动及神经功能恢复,对改善患者肢体运动功能及预后有积极意义。

Objective To observe the effect of the acupuncture approach of coordinating the body and spirit combined with an early rehabilitation on improving limb movement at post-stroke.Methods It was a prospective study involving 110 stroke patients admitted in the Yellow River Sanmenxia Hospital between May 2021 and May 2023.They were randomly divided into the conventional group(55 cases)and combined group(55 cases)using computer randomization.Patients in the conventional group received routine treatment and early rehabilitation training,while those in the combined group additionally received acupuncture approach of coordinating the body and spirit.The therapeutic effects were compared between groups.Results After treatment,α,β and θ wave frequencies on the electroencephalogram(EEG)scan were significantly lower in the combined group than those of the conventional group(P<0.05).After treatment,NSE and S100β levels were significantly lower in the combined group than those of the conventional group(P<0.05).After treatment,the national institutes of health stroke scale(NIHSS)score was significantly lower in the combined group than that of the conventional group,and the modified Barthel index(MBI)was significantly higher(P<0.05).After treatment,the simplified Fugl-Meyer assessment(FMA)scores of the upper and lower limbs were significantly higher in the combined group than those of the conventional group(P<0.05).After follow-up for 6 months,the incidence of adverse events in the combination group was significantly lower in the combined group than that of the conventional group(7.27% [4/55] VS 21.82% [12/55],P<0.05).Conclusion Acupuncture approach of coordinating the body and spirit combined with an early rehabilitation is beneficial to the recovery of cerebral electrophysiological activity and neurological function in post-stroke patients,offering positive significances in improving limb motor function and prognosis.
