自由体位与常规卧位对无痛分娩产妇分娩结局的影响

Effect of free position and conventional position on delivery outcomes of women in painless labor

目的 探讨自由体位与常规卧位对无痛分娩产妇分娩结局的影响。方法 选取90名接受无痛分娩产妇,根据随机数字表法分组,每组45名。对照组予以常规卧位分娩,研究组予以自由体位分娩。比较两组产妇分娩方式、产程时间与产程干预情况。结果 较对照组,研究组自然分娩率更高,剖宫产率更低(P<0.05)。较对照组,研究组各产程时间更短(P<0.05)。较对照组,研究组人工破膜、使用催产素、会阴侧切与阴道助产率均更低(P<0.05)。结论 对无痛分娩产妇采取自由体位分娩,较常规卧位分娩能够明显提高产妇自然分娩率,降低剖宫产率,且可以加快产妇的产程并减少产程中的人工干预措施,值得临床推广。

Objective To explore the effect of free position and conventional position on the delivery outcome of women in painless labor.Methods 90 women receiving painless delivery were selected and divided into 45 women in each group according to random number table method.The control group was given conventional decubitus position delivery,and the study group was given free position delivery.Methods of delivery,labor time and labor intervention were compared between the two groups.Results Compared with the control group,the study group had a higher rate of natural delivery and a lower rate of cesarean section(P<0.05).Compared with the control group,the duration of labor in the study group was shorter(P<0.05).Compared with the control group,the rates of artificial rupture of membranes,use of oxytocin,lateral perineal incision and vaginal midbirth were lower in the study group(P<0.05).Conclusion Compared with conventional decubitus delivery,free position delivery for women with painless delivery can significantly increase the rate of natural delivery,reduce the rate of cesarean section,and speed up the labor process and reduce the manual intervention measures during labor,which is worthy of clinical promotion.
