超选择子宫动脉药物灌注联合栓塞治疗内生型剖宫产瘢痕妊娠的临床效果

Clinical effect of superselective uterine artery drug infusion combined with embolization in the treatment of endogenous type cesarean scar pregnancy

目的探讨超选择子宫动脉药物灌注联合栓塞治疗内生型剖宫产瘢痕妊娠的临床效果。方法选取2020年8月至2023年1月在内蒙古科技大学包头医学院第一附属医院诊治的78例内生型剖宫产瘢痕妊娠患者作为研究对象,根据随机数字表法分为联合组(39例)与对照组(39例)。对照组给予双侧子宫动脉栓塞治疗,联合组在对照组的基础上给予超选择子宫动脉药物灌注治疗,比较两组的临床效果。结果联合组月经恢复时间、β-人绒毛膜促性腺激素(β-HCG)水平恢复正常时间较对照组短(P<0.05);治疗3个月后联合组子宫动脉血流搏动指数、阻力指数高于对照组(P<0.05);联合组不良反应总发生率低于对照组(P<0.05)。结论超选择子宫动脉药物灌注联合栓塞治疗内生型剖宫产瘢痕妊娠,能够促进月经及β-HCG恢复,改善子宫动脉血流动力学,降低不良反应发生率。

Objective To investigate the clinical effect of superselective uterine artery drug infusion combined with embolization in the treatment of endogenous type cesarean scar pregnancy.Methods A total of 78 patients with endogenous type cesarean scar pregnancy diagnosed and treated in the First Affiliated Hospital of Baotou Medical College,Inner Mongolia University of Science&Technology from August 2020 to January 2023 were selected as the research objects.According to the random number table method,they were divided into the combined group(39 cases)and the control group(39 cases).The control group was treated with bilateral uterine artery embolization,and the combined group was treated with superselective uterine artery drug infusion on the basis of the control group.The clinical effect of the two groups was compared.Results The recovery time of menstruation and the recovery time ofβ-human chorionic gonadotropin(β-HCG)level to normal in the combined group were shorter than those in the control group(P<0.05).After 3 months of treatment,the uterine artery blood flow pulsatility index and resistance index of the combined group were higher than those of the control group(P<0.05).The total incidence of adverse reactions in the combined group was lower than that in the control group(P<0.05).Conclusion Superselective uterine artery drug infusion combined with embolization in the treatment of endogenous type cesarean scar pregnancy can promote the recovery of menstruation andβ-HCG,improve uterine artery hemodynamics,and reduce the incidence of adverse reactions.
