摘要
目的研究胸椎经棘突后正中椎管切开复位术的安全性及有效性。方法将6具新鲜冰冻标本的胸椎逐个节段沿棘突后正中切开,使用专用椎板撑开器撑开缝宽8mm、10mm、12mm后测量。3具新鲜冰冻标本模拟胸椎经棘突后正中椎管切开复位术,显微镜下观察暴露范围和术区结构。结果撑开缝宽12mm以内为实验所得的安全距离,无骨折发生。缝宽12mm手术操作空间足够,器械自由度较高,可以进行椎管内肿瘤的切除。结论胸椎经棘突后正中椎管切开复位术治疗胸椎椎管内肿瘤是一种安全、有效的方式。
Objective To study the safety and effectiveness of the split laminotomy of thoracic vertebrae via posterior midline spinous process.Methods Thoracic vertebrae of 6 fresh frozen specimens were splited segment by segment via the posterior midline spinous process,and the width of the suture was 8mm,10mm,and 12mm using a special lamina opener.Three fresh frozen specimens simulated the split laminotomy of thoracic vertebrae via posterior midline spinous process,and the exposure range and surgical area structure were observed under the microscope.Results The expansion slit width was within 12mm,and the safety distance obtained by the experiment was not fracture.The surgical operation space with a suture width of 12mm was sufficient,and the degree of instrument freedom was high,and the tumor in the spinal canal can be removed.Conclusion The split laminotomy of thoracic vertebrae via posterior midline spinous process is a safe and effective way to treat tumors in the thoracic spinal canal.
