摘要
目的探究胰岛素泵与胰岛素多次注射对妊娠期糖尿病患者的治疗效果与安全性。方法选取本院24例妊娠期糖尿病患者为研究对象,按照随机数字表法分为两组,各12例。多次注射组患者采取多次皮下注射胰岛素治疗,胰岛素泵组患者采取持续皮下胰岛素输注治疗。比较两组患者用药前后的血糖水平、用药期间低血糖发生率及妊娠结局。结果用药前,两组患者血糖水平比较差异无统计学意义(P>0.05)。用药后,胰岛素泵组患者的空腹血糖、餐后2h血糖、糖化血红蛋白水平明显低于多次注射组患者,差异有统计学意义(P<0.05)。两组患者胎膜早破发生率比较差异无统计学意义(P>0.05)。胰岛素泵组患者用药期间低血糖发生率低于多次注射组患者,自然分娩率高于多次注射组,差异有统计学意义(P<0.05)。结论相较于胰岛素多次注射给药,胰岛素泵给药能够有效控制妊娠期糖尿病患者的血糖水平,不良反应发生率更低,可以提高自然分娩率,具有较高的临床应用与推广价值。
Objective To explore the efficacy and safety of insulin pump and multiple insulin injections in the treatment of pregnant women with diabetes.Methods 24 patients with gestational diabetes in our hospital were selected as research objects.Divide into two groups according to the random number table method,with 12 cases in each group.Patients in the multiple injection group received multiple subcutaneous insulin injections for treatment,while patients in the insulin pump group received continuous subcutaneous insulin infusion for treatment.Compare the blood glucose levels,incidence of hypoglycemia during medication,and pregnancy outcomes between the two groups of patients before and after medication.Results Before medication,there was no statistically significant difference in blood glucose levels between the two groups of patients(P>0.05).After medication,the levels of FBG,2 h PG,and HbA1c in the insulin pump group were significantly lower than those in the multiple injection group,with statistical significance(P<0.05).There was no statistically significant difference in the incidence of premature rupture of membranes between the two groups of patients(P>0.05).The incidence of hypoglycemia during medication in the insulin pump group was lower than that in the multiple injection group,and the natural delivery rate was higher than that in the multiple injection group,with a statistically significant difference(P<0.05).Conclusion Compared with multiple injections of insulin,insulin pump administration can effectively control the blood sugar level of pregnant women with diabetes,and the incidence of adverse reactions is lower.It can improve the natural delivery rate,and has higher clinical application and promotion value.
