酒精依赖患者戒断期血浆食欲素A浓度变化及其影响因素的相关性研究

Correlation Study of Plasma Orexin A Concentration and its Influencing Factors in Alcohol-dependent Patients During Withdrawal Period

目的:探讨酒精依赖患者戒断期血浆食欲素A(OXA)浓度以及相关影响因素。方法:使用修订版临床酒精戒断症状评估量表中文版(clinic institute alcohol withdrawal syndrome scale,CIWA-Ar)、抑郁自评量表(self-rating depression scale,SDS)、焦虑自评量表(self-rating anxiety scale,SAS)、匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)、自制饮酒问卷等评估86例酒精依赖患者的戒断严重程度、焦虑抑郁症状、睡眠质量和饮酒使用情况,并对比86例酒精依赖患者以及85例正常对照组血浆OXA水平之间的差异性,进一步分析酒精依赖戒断期患者OXA水平的影响因素。结果:酒精依赖患者组在戒断期血浆OXA水平高于正常对照组(t=5.286,P<0.05),而酒精依赖患者血浆OXA水平与CIWA-Ar评分(r=0.405,P<0.05)、睡眠质量(r=0.479,P<0.05)、SAS(r=0.316,P<0.05)、SDS(r=0.404,P<0.05)均呈显著性正相关。进一步进行多因素分析显示,CIWA-Ar、PSQI分值、SDS是影响血浆OXA水平的关键因素。结论:酒精依赖患者戒断期血浆OXA水平较高,戒断症状、睡眠质量以及焦虑情绪均会影响血浆OXA水平。

Objective:To investigate the plasma orexin-A(OXA)concentration and related influencing factors in alcohol dependent patients during withdrawal.Methods:The clinical institute alcohol withdrawal syndrome scale(CIWA-Ar),self-rating depression scale(SDS),self-rating anxiety scale(SAS),Pittsburgh sleep quality index(PSQI)and self-made drinking questionnaire were used to evaluate the withdrawal severity,anxiety and depression symptoms,sleep quality and drinking use of 86 patients with alcohol dependence.The differences of plasma OXA levels between 86 patients with alcohol dependence and 85 normal controls were compared,and the influencing factors of OxA levels in patients with alcohol dependence withdrawal period were further analyzed.Results:The level of plasma OXA in patients with alcohol dependence during withdrawal period was higher than that in normal control group(t=5.286,P<0.05),while the level of plasma OXA in patients with alcohol dependence was positively correlated with CIWA-Ar score(r=0.405,P<0.05),sleep quality(r=0.479,P<0.05),SAS(r=0.316,P<0.05)and SDS(r=0.404,P<0.05).Further multivariate analysis showed that CIWA-Ar,PSQI score and SDS were the key factors affecting plasma OXA level.Conclusion:The level of plasma OXA in patients with alcohol dependence during withdrawal is higher.Withdrawal symptoms,sleep quality,and anxietycanaffectplasmaOXAlevels.
