石榴补血糖浆联合蔗糖铁治疗术后缺铁性贫血的临床研究

Clinical study of Shiliu Buxue Syrup combined with iron sucrose in treatment of postoperative iron deficiency anemia

目的探讨石榴补血糖浆联合蔗糖铁治疗术后缺铁性贫血的临床疗效。方法选取2021年3月—2023年3月上海中医药大学附属曙光医院收治的96例术后缺铁性贫血患者,按照随机数字表法分对照组和治疗组,每组各48例。对照组患者静脉滴注蔗糖铁注射液,0.9%氯化钠注射液100 mL加入蔗糖铁注射液100 mg,3次/周。治疗组在对照组治疗基础上口服石榴补血糖浆,20 mL/次,3次/d。两组用药30 d。观察两组的临床疗效和症状缓解时间,比较两组治疗前后血常规指标和血清炎性因子的变化情况。结果治疗后,治疗组总有效率是97.92%,显著高于对照组的81.25%(P<0.05)。治疗后,治疗组乏力、心悸、头晕、皮肤黏膜苍白改善时间均显著短于对照组(P<0.05)。治疗后,两组患者血红蛋白(HGB)、血清铁蛋白(SF)、转铁蛋白饱和度(TS)、血细胞比容(HCT)水平均较同组治疗前显著升高(P<0.05);治疗后,治疗组的HGB、SF、TS、HCT水平均高于对照组(P<0.05)。治疗后,两组患者血清白细胞介素-6(IL-6)、膜结合丝氨酸蛋白酶-2(MTPS-2)、血清铁调素(Hepcidin)、肿瘤坏死因子-α(TNF-α)水平均较同组治疗前显著降低(P<0.05);治疗后,治疗组IL-6、MTPS-2、Hepcidin、TNF-α水平均显著低于对照组(P<0.05)。结论石榴补血糖浆联合蔗糖铁治疗术后缺铁性贫血有较好的临床疗效,可明显改善患者贫血状态,提高机体内铁水平,减弱术后炎性反应,值得借鉴应用。

Objective To explore the clinical efficacy of Shiliu Buxue Syrup combined with iron sucrose in treatment of postoperative iron deficiency anemia.Methods Ninety-six patients with postoperative iron deficiency anemia admitted to Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from March 2021 to March 2023 were selected and divided into control group and treatment group according to random number table method,with 48 cases in each group.Patients in the control group were iv administered with Iron Sucrose Injection,0.9%sodium chloride injection 100 mL added Iron Sucrose Injection 100 mg,3 times weekly.Patients in the treatment group were po administered with Shiliu Buxue Syrup on the basis of the control group,20 mL/time,3 times daily.Both groups were treated for 30 d.The clinical efficacy and symptom remission time of the two groups were observed,and the changes of blood routine indexes and serum inflammatory factors before and after treatment were compared.Results After treatment,the total effective rate of treatment group was 97.92%,which was significantly higher than that of control group(81.25%,P<0.05).After treatment,the improvement time of fatigue,palpitation,dizziness,and pale skin mucosa in treatment group was significantly shorter than that in control group(P<0.05).After treatment,the levels of hemoglobin(HGB),serum ferritin(SF),transferrin saturation(TS),and hematocrit(HCT)in 2 groups were significantly increased compared with before treatment(P<0.05).After treatment,the HGB,SF,TS,and HCT levels in the treatment group were higher than those in control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),membrane-bound serine proteinase-2(MTPS-2),serum ferritin(Hepcidin),and tumor necrosis factor-α(TNF-α)in 2 groups were significantly decreased compared with those before treatment(P<0.05).After treatment,the levels of IL-6,MTPS-2,Hepcidin,and TNF-αin the treatment group were significantly lower than those in control group(P<0.05).Conclusion Shiliu Buxue Syrup combined with iron sucrose has good clinical effect in treatment of postoperative iron deficiency anemia,and can obviously improve the anemia state of patients,increase the iron level in the body,which can weaken the postoperative inflammatory response.
