黄芪桂枝五物汤加减治疗气虚血瘀型糖尿病周围神经病变的效果观察

Effect of Huangqi Guizhi Wuwu Decoction on Peripheral Neuropathy of Qi-deficiency and Blood-stasis Type Diabetes Mellitus

目的:观察黄芪桂枝五物汤加减治疗气虚血瘀型糖尿病周围神经病变的效果。方法:选取2021年3月—2022年6月北京市朝阳区金盏第二社区卫生服务中心收治的气虚血瘀型糖尿病周围神经病变患者94例作为研究对象,采取随机数字表法分为对照组与观察组,各47例。对照组采用常规西药治疗,观察组在对照组基础上采用黄芪桂枝五物汤治疗。比较两组治疗效果、临床症状评分、腓总神经传导速度、血糖水平。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.006)。治疗后,两组肢体疼痛、肢体麻木、乏力评分降低,观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组运动神经传导速度、感觉神经传导速度加快,观察组快于对照组,差异有统计学意义(P<0.05)。治疗后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平降低,观察组低于对照组,差异有统计学意义(P<0.05)。结论:黄芪桂枝五物汤加减治疗气虚血瘀型糖尿病周围神经病变的效果理想,能减轻症状,提高腓总神经传导速度,改善血糖指标。

Objective:To observe the effect of Huangqi Guizhi Wuwu Decoction on peripheral neuropathy of Qi-deficiency and blood-stasis type diabetes mellitus.Methods:A total of 94 patients with Qi-deficiency and blood-stasis type diabetic peripheral neuropathy were selected as the study objects at Jinjiao Second Community Health Service Center,Chaoyang District,Beijing from March 2021 to June 2022,and were divided into control group and observation group by random number table method,with 47 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Huangqi Guizhi Wuwu Decoction based on the control group.The therapeutic effect,clinical symptom score,common peroneal nerve conduction velocity and blood glucose level were compared between the two groups.Results:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P=0.006).After treatment,the scores of limb pain,limb numbness and fatigue of the two groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,the motor nerve conduction speed and sensory nerve conduction speed of the two groups were accelerated,and the observation group was faster than the control group,the difference was statistically significant(P<0.05).After treatment,the levels of fasting blood glucose,2 h postprandial blood glucose and glycated hemoglobin in the two groups were decreased,and the observation group was lower than the control group,with statistical significance(P<0.05).Conclusion:Huangqi Guizhi Wuwu Decoction is effective in treating Qi-deficiency and bloodstasis type diabetes peripheral neuropathy,which can alleviate symptoms,increase the common peroneal nerve conduction speed and improve blood glucose index.
