宫颈癌患者术后感染的病原学特点、易感因素分析

Etiological characteristics and predisposing factors of postoperative infection in patients with cervical cancer

摘要 目的探究宫颈癌患者术后感染的病原学特点、易感因素。方法选例时间:2022年3月-2023年3月,回顾性收集沙洋县妇幼保健院收治的100例临床资料(宫颈癌患者),其中感染组:21例术后感染患者临床资料,未感染组:79例术后无感染患者临床资料,分组依据:宫颈癌患者术后感染情况;分析感染组患者病原菌分布情况,归纳影响宫颈癌患者术后感染的危险因素。结果21例感染组患者共检出病原菌34株,其中革兰阴性菌18株,革兰阳性菌13株,真菌3株;经单因素、多因素分析结果显示:年龄≥60岁、尿潴留、血清白蛋白浓度<30g/L、合并糖尿病均为宫颈癌患者术后感染的独立危险因素(OR=2.423、2.155、2.396、2.121,P<0.05)。结论宫颈癌术后感染的病原菌以革兰阴性菌为主,同时年龄≥60岁、尿潴留、血清白蛋白浓度<30g/L、合并糖尿病均为宫颈癌患者术后感染的独立危险因素,临床可据此予以早期诊疗,以降低宫颈癌术后感染的发生风险。 Objective To explore the pathogenic characteristics and susceptibility factors of postoperative infection in patients with cervical cancer.Methods From March 2022 to March 2023,the clinical data of 100 patients with cervical cancer admitted to our hospital were retrospectively collected.The infection group included 21 patients with postoperative infection,and the non-infection group included 79 patients without postoperative infection.The distribution of pathogenic bacteria in the infection group was analyzed,and the risk factors affecting postoperative infection in patients with cervical cancer were summarized.Results A total of 34 strains of pathogenic bacteria were detected in 21 patients with infection,including 18 strains of gram-negative bacteria,13 strains of Gram-positive bacteria,and 3 strains of fungi.The results of univariate and multivariate analysis showed that age≥60 years,urinary retention,serum albumin concentration<30g/L,and diabetes mellitus were independent risk factors for postoperative infection in patients with cervical cancer(OR=2.423,2.155,2.396,2.121,P<0.05).Conclusions Gram-negative bacteria are the main pathogens of postoperative infection in patients with cervical cancer.Age≥60 years,urinary retention,serum albumin<30g/L,and diabetes mellitus are independent risk factors for postoperative infection in patients with cervical cancer.Early diagnosis and treatment can be given to reduce the risk of postoperative infection in patients with cervical cancer.
