摘要
本试验对在北京市西部山区调查发现的1例犬利什曼原虫病临床疑似病例开展了诊断和分析,采用血液检查、淋巴穿刺液显微镜检查、血清胶体金抗体检测、皮屑样品荧光定量PCR检测以及核糖体DNA转录间隔区I(ITS-1)基因扩增和测序分析对病例开展全面诊断。结果显示,该病例红细胞数、血红蛋白、红细胞压积、淋巴细胞百分比、白蛋白、碱性磷酸酶低于参考范围,球蛋白、钙离子、肌酸激酶高于参考范围,呈现寄生虫感染的典型特征;显微镜下可以观察到明显利什曼原虫虫体;胶体金抗体检测和荧光定量PCR检测结果均为阳性;基因序列分析结果显示本病例ITS-1基因与以色列婴儿利什曼原虫KM677134.1聚为一支。结合地理来源,表明该地区流行株为山丘型婴儿利什曼原虫。
This study conducted diagnosis and analysis of a suspected case of canine leishmaniasis discovered during a survey in the western mountainous area of Beijing.Comprehensive diagnosis of the case was carried out using blood examination,microscopic examination of lymphatic puncture fluid,serum colloidal gold antibody testing,fluorescence quantitative PCR testing of skin scraping samples,and amplification and sequencing analysis of the ribosomal DNA internal transcribed spacer 1(ITS-1)gene.The results showed that the case exhibited typical features of parasitic infection,with red blood cell count,hemoglobin,hematocrit,lymphocyte percentage,albumin,and alkaline phosphatase below the reference range,while globulin,calcium,and creatine kinase were above the reference range.Leishmania polypides were clearly observed under the microscope.Both colloidal gold antibody testing and fluorescence quantitative PCR testing yielded positive results.Sequence analysis of the gene revealed that the ITS-1 gene of this case clustered with Leishmania infantum Israel KM677134.1.Combined with geographic origin,it indicates that the prevalent strain in the area is the mountainous Leishmania infantum.
