肿瘤医院护士濒死期护理实践行为现状及影响因素研究

The study of status quo and influencing factors on nursing practice behavior of near-death period among nurses in tumor hospital

目的调查肿瘤医院护士濒死期护理实践行为现状,分析其影响因素。方法选取2023年7月至8月某肿瘤医院护士462名为研究对象,采用一般资料问卷、濒死期护理实践行为评价问卷、安宁疗护护理质量量表及护士悲伤状态量表对其进行调查,采用多元线性回归分析肿瘤医院护士濒死期护理实践行为影响因素。结果肿瘤医院护士濒死期护理实践行为得分为(62.70±9.16)分,安宁疗护护理质量得分为(65.80±11.40)分,悲伤状态得分为(51.72±7.64)分,肿瘤医院护士濒死期护理实践行为总分与安宁疗护护理质量总分呈正相关(r=0.446,P<0.01);与悲伤状态总分呈负相关(r=0.442,P<0.01)。多元线性回归分析显示,肿瘤医院护士的职称、宗教信仰、安宁疗护护理质量和悲伤状态是肿瘤医院护士濒死期护理实践行为的影响因素(P<0.01),可解释总变异的40.2%。结论肿瘤医院护士濒死期护理实践行为处于中等水平,护理管理者应重视低学历、低职称护士的濒死期护理实践能力,通过临终关怀及死亡相关教育引导护士客观认识死亡,树立正确的死亡观,提升肿瘤医院护士的职业价值感和职业认同感,更好地为濒死期患者实施护理。

Objective To investigate the status quo of nursing practice behavior of nurses in cancer hospital in the near-death period,and analyze its influencing factors.Methods By using convenience sampling method,462 nurses in a cancer hospital from July to August 2023 were selected as the research subjects.A survey was conducted by using general information questionnaire,evaluation questionnaire on nursing practice behavior during the near-death period,palliative care quality scale and nurse sadness state scale.By used multiple linear regression analysis to investigate the influencing factors of nursing practice behavior among tumor hospital nurses during the near-death period.Results The score of nursing practice behavior among nurses in tumor hospitals was(62.70±9.16),the score of quality of hospice care was(65.80±11.40),and the score of sadness state was(51.72±7.64).The total score of nursing practice behavior among nurses in tumor hospitals was positively correlated with the total score of quality of hospice care(r=0.446,P<0.01).There was a negative correlation with the total score of sadness state(r=0.442,P<0.01).The multiple linear regression analysis showed that the professional title,religious belief,quality score of hospice care,and sadness state of nurses in cancer hospitals were the main influencing factors(P<0.01)for the score of nursing practice behavior in the dying stage,which could explain 40.2%of the total variance.Conclusion The nursing practice behavior of nurses in tumor hospital is at middle level.Nursing managers should pay attention to the nursing practice behavior of low education and professional title nurses,through palliative care and death-related education,nurses are guided to understand death objectively,establish a correct view of death,enhance the sense of professional value and professional identity of nurses in tumor hospitals,and better to carry out nursing care for patients at the dying stage.
