功能性鼻内镜手术九步法标准化疗法对慢性鼻窦炎患者疼痛程度和鼻内镜Lund-Kennedy评分与鼻窦CT

Effects of nine-step standardized treatment for functional endoscopic nasal surgery on pain degree,nasal endoscopic Lund-Kennedy score,sinus CT Lund-Mackay score and quality of life in patients with chronic sinusitis

目的探究功能性鼻内镜手术九步法标准化疗法对慢性鼻窦炎患者疼痛程度、鼻内镜Lund-Kennedy评分、鼻窦CTLund-Mackay评分及生命质量的影响。方法选取2019年1月至2021年6月于吉安市中心人民医院就诊的82例慢性鼻窦炎患者作为研究对象,按照随机抽签法分为对照组与观察组,每组41例。对照组采用传统开放式手术,观察组采用功能性鼻内镜手术九步法标准化疗法,比较两组临床疗效、疼痛程度、病情严重程度、鼻窦恢复情况、并发症发生情况及生命质量。结果观察组治疗总有效率为95.12%,高于对照组的78.05%,差异有统计学意义(P<0.05)。术后6个月,两组视觉模拟评分法(VAS)、鼻窦CTLund-Mackay、鼻内镜Lund-Kennedy评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率为4.88%,低于对照组的21.95%,差异有统计学意义(P<0.05)。术后6个月,两组鼻腔鼻窦结局测试-20(SNOT-20)评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论与传统开放式手术比较,功能性鼻内镜手术九步法标准化疗法治疗慢性鼻窦炎的近期效果较理想,可减轻患者疼痛及病情严重程度,促进鼻窦恢复,提高患者生命质量,且安全性高,可作为慢性鼻窦炎患者的理想治疗术式之一,值得临床推广应用。

Objective To explore the effects of nine-step standardized treatment of functional endoscopic nasal surgery on pain degree,nasal en-doscopic Lund-Kennedy score,sinus CT Lund-Mackay score and quality of life in patients with chronic sinusitis.Methods A total of 82 patients with chronic sinusitis treated in Ji'an Central People's Hospital from January 2019 to June 2021 were selected as the study subjects,and they were divided into the control group and the observation group according to the random drawing method,with 41 cases in each group.The control group was treated with traditional open surgery,and the observation group was treated with nine-step standardized treatment of functional endoscopic nasal surgery,the clinical efficacy,pain degree,severity of disease,sinus recovery,complications and quality of life were compared between the two groups.Results The total effective rate in the observation group was 95.12%,which was higher than 78.05%in the control group,the difference was statistically significant(P<0.05).6 months after surgery,the visual analogue scale(VAS)score,nasal endoscopic Lund-Kennedy score and si-nus CT Lund-Mackay score of the two groups were lower than before surgery,and the observation group was lower than the control group,the dif-ferences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 4.88%,which was lower than 21.95%in the control group,and the difference was statistically significant(P<0.05).6 months after surgery,the sino-nasaloutcometest-20(SNOT-20)scores of the two groups were lower than those before surgery,and the observation group was lower than the control group,the differ-ence was statistically significant(P<0.05).Conclusion Compared with traditional open surgery,nine-step standardized treatment of functional en-doscopic nasal surgery has ideal short-term effects in treatment of chronic sinusitis,which can reduce the pain and severity of the disease,promote the recovery of paranasal sinuses,improve the quality of life of patients,and has high safety.it can be used as one of the ideal treatment methods for patients with chronic sinusitis,which is worthy of clinical promotion and application.
