伤科一号方穴位贴敷联合热敏灸对膝骨关节炎疼痛及膝关节功能的影响

Effects of the No.1 Shangke prescription acupoint application combined with thermosensitive moxibustion on pain and knee joint function of knee osteoarthritis

目的观察伤科一号方穴位贴敷联合热敏灸治疗膝骨关节炎(KOA)的效果。方法将88例KOA患者按随机数字表法分为对照组和研究组,各44例。对照组给予常规治疗,研究组在对照组基础上给予伤科一号方穴位贴敷联合热敏灸治疗。比较两组的治疗效果。结果研究组的治疗总有效率高于对照组(P<0.05)。治疗后,两组的视觉模拟评分法(VAS)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)、奎森功能演算指数(Lequesne评分)、前列腺素E2、P物质、5-羟色胺、Ⅰ型胶原C-末端肽交联(CTX-Ⅰ)、软骨寡聚基质蛋白(COMP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)及白细胞介素-17(IL-17)水平均低于治疗前,且研究组低于对照组(P<0.05)。治疗后,两组的骨钙素、骨保护素水平均高于治疗前,且研究组高于对照组(P<0.05)。结论伤科一号方穴位贴敷联合热敏灸能改善KOA患者的膝关节功能,缓解疼痛,调节骨代谢指标,降低炎症因子水平,提高临床效果。

Objective To observe the effect of No.1 Shangke prescription acupoint application combined with thermosensitive moxibustion in the treatment of knee osteoarthritis(KOA).Methods According to the random number table method,88 patients with KOA were divided into control group and study group,with 44 cases in each group.The control group was given routine treatment,and the study group was given the No.1 Shangke prescription acupoint application combined with thermosensitive moxibustion on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).After treatment,the Visual Analogue Scale(VAS),the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC),Lequesne scores,prostaglandin E2,substance P,5-hydroxytryptamine,C-terminal telopeptide of typeⅠcollagen(CTX-Ⅰ),cartilage oligomeric matrix protein(COMP),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-17(IL-17)levels in the two groups were lower than those before treatment,and those in the study group were lower than the control group(P<0.05).After treatment,the osteocalcin and osteoprotegerin levels in the two groups were higher than those before treatment,and those in the study group were higher than the control group(P<0.05).Conclusion The No.1 Shangke prescription acupoint application combined with thermosensitive moxibustion can improve the knee joint function of KOA patients,relieve pain,regulate bone metabolism indexes,reduce the level of inflammatory factors,and improve the clinical effect.
