柴胡加龙骨牡蛎汤加减与柴胡疏肝散治疗肝郁气滞型郁病患者的效果比较

Comparison of effects of modified Chaihu Jia Longgu Muli decoction and Chaihu Shugan powder in treatment of patients with depression of liver qi stagnation type

目的:比较柴胡加龙骨牡蛎汤加减与柴胡疏肝散治疗肝郁气滞型郁病患者的效果。方法:选取2023年1—5月该院收治的120例肝郁气滞型郁病患者进行前瞻性研究,按照随机数字表法将其分为观察组与对照组各60例。对照组采取柴胡疏肝散治疗,观察组采取柴胡加龙骨牡蛎汤加减治疗,比较两组治疗总有效率、抑郁自评量表(SDS)评分、汉密尔顿抑郁量表(HAMD)评分、Barthel指数(BI)评分、中医证候总积分和不良反应发生率。结果:观察组治疗总有效率为96.67%,明显高于对照组的80.00%,差异有统计学意义(P<0.05);治疗后,两组BI评分高于治疗前,且观察组高于对照组,两组SDS、HAMD评分以及中医证候总积分低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:柴胡加龙骨牡蛎汤加减治疗肝郁气滞型郁病患者可提高治疗总有效率和BI评分,以及降低SDS评分、HAMD评分和中医证候总积分,效果优于柴胡疏肝散治疗。

Objective:To compare effects of modified Chaihu Jia Longgu Muli decoction and Chaihu Shugan powder in treatment of patients with depression of liver qi stagnation type.Methods:A prospective study was conducted on 120 patients with depression of liver qi stagnation type admitted to this hospital from January to May 2023.According to the random number table method,they were divided into observation group and control group,60 cases in each group.The control group was treated with Chaihu Shugan powder,while the observation group was treated with Chaihu Jia Longgu Muli decoction.The total effective rate of treatment,the self-rating depression scale(SDS)score,the Hamilton depression scale(HAMD)score,the Barthel index(BI)score,the total score of TCM syndromes,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 96.67%,which was significantly higher than 80.00%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the BI scores of the two groups were higher than those before the treatment,and that in the observation group was higher than that in the control group;the SDS and HAMD scores as well as the total scores of TCM syndromes in the two groups were lower than those before the treatment,and those in the observation group were lower than those in the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Chaihu Jia Longgu Muli decoction can improve the total effective rate and the BI scores,and reduce the SDS scores,the HAMD scores and the total score of TCM syndromes in the treatment of the patients with depression of liver qi stagnation type.Moreover,it is superior to Chaihu Shugan powder treatment.
